2622株泌尿系感染病原菌的分布及耐药性分析

【摘要】 目的收集我院2008-2010年尿培养阳性菌株2622例,分析病原菌的分布和耐药状况,帮助临床合理有效的选用抗生素。方法对尿液培养中分离出的2622株病原菌使用VITEK TOW自动微生物鉴定系统进行鉴定和药敏实验,并对ESBL菌株进行耐药监测。结果在引起泌尿系感染的病原菌中,大肠埃希菌检出率最高为32.38%,其次为念珠菌17.47%,肠球菌14.72%。在药敏分析中,ESBL+肠杆菌科细菌对亚胺培南耐药率最低,哌拉西林/他唑巴坦,阿米卡星,头孢匹肟,头孢哌酮/舒巴坦耐药率较低,也可选用。葡萄球菌对糖肽类的耐药率均为0,对呋喃妥因,磷霉素等耐药率低。肠球菌对利奈唑胺、替考拉宁、万古霉素的耐药率较低。结论在导致泌尿系感染的病原菌中,以大肠埃希菌、真菌和肠球菌为主,细菌的耐药率呈上升趋势。细菌耐药监测为临床合理使用抗生素提供依据。

【Abstract】 Objective To collected 2622 strains isolated from urine since year 2008 to 2010 in people’s hospital.To analyze the pathogenic bacteria’s distribution and drug resistance and to help clinicians using antibiotics reasonably and effectively.Methods 2622 strains of pathogens were identified and the antibiotics resistance was tested by VITEK TWO system and monitored antibiotic resistance to ESBL+ strains.Results In the pathogens of the urinary tract infection,The Escherichia coli reached the top(32.38%),secondly were Candida albicans(17.47%) and Enterococcus(14.72%).The antibiotics resistance of ESBL+ Enterobacteriaceae,to imipenem was the lowest,Cefoperazone-sulbact,Piperacillin-tazobactam,Amikacin,Cefepime were the lower.Staphylococcus have no resistance to Glycopeptides and a few to Nitrofurantoin,fosfomycin.In Enterococcus the resistance rate to Linezolid,Vancomycin,,Teicoplanin were the lower.Conclusion Escherichia coli,Candida albicans and Enterococcus are the main pathogens to urinary tract infections.and drug resistance tends to rise.Monitoring the antibiotics resistance could afford pegs to the reasonable application of antibiotics.
