【摘要】 目的观察乌司他丁联合血必净治疗烧伤脓毒症的临床效果。方法遴选68例烧伤脓毒症患者并随机分为实验组36例和对照组32例。烧伤脓毒症确诊后,对照组按烧伤脓毒症综合治疗方案进行治疗,实验组在烧伤脓毒症综合治疗方案治疗的基础上加用乌司他丁100kU静脉注射,2次/d和血必净注射液100ml静脉滴注,2次/d,连续应用7d。比较两组用药治疗前后7d的痂下组织细菌定量的差异;比较用药治疗7d后,两组血培养阴转率、脓毒症持续时间、多脏器功能不全综合征发生率和生存率的差异。结果联合用药7d后,实验组痂下组织细菌定量(4.16±1.48)×103CFU/g明显低于对照组(5.89±0.36)×107CFU/g(P<0.05);实验组血细菌培养阳性阴转率(76.2%)高于对照组(27.8%)(P<0.05);实验组烧伤脓毒症持续时间(5.1±0.8)d较对照组(7.9±1.0)d缩短(P<0.01),多脏器功能不全综合征发生率16.7%较对照组40.6%降低(P<0.05),生存率(75.0%)高于对照组(46.9%)(P<0.05)。结论乌司他丁联合血必净治疗烧伤脓毒症,可抑制机体内的细菌生长,缩短烧伤脓毒症持续时间,降低MODS的发生,提高烧伤患者的生存率。更多还原

【Abstract】 s] Objective To investigate the clinical effects of ulinastain and xuebijing on the sepsis of burns.Methods Sixty-eight burn patients with sepsis were divided randomly into two groups:treatment group(n=36) and control group(n=32).The control group was medicated with comprehensive treatment of burn sepsis and the treatment group with ulinastain 100 kU iv b.i.d and xuebijing injection 100 ml iv drip b.i.d besides the control group’,when sepsis of burns diagnosed.The comparison of subeschar tissue bacterial quantitative culture,the negative rate of bacterial growth in blood,the lasting time of sepsis,the occurrence of MODS and survival rate were made.Results After 7 days,the subeschar tissue bacterial counting of treatment group was lower significantly than that of control group [(4.16±1.48)×103 CFU/g vs(5.89±0.36)×107 CFU/g,P<0.05],lasting time of sepsis [(5.1±0.8)d vs(7.9±1.0)d,P<0.01] and the occurrence of MODS(16.7% vs 40.6%,P<0.05)of treatment group were all lower significantly,the negative rate of bacterial growth in blood(76.2% vs 27.8%,P<0.05) and survival rate(75.0% vs 46.9%,P<0.05)were higher than that of control group.Conclusion The treatment with ulinastain and xuebijing on burn sepsis could inhibit the bacterial growth in the body,shorten the duration of burn sepsis,decrease the occurrence of MODS and increase the survival rate of burn patients.更多还原
