小儿先天性心脏病术前脑利钠肽检测及心功能维护的研究

【摘要】 目的:通过检测小儿先天性心脏病(CHD)心脏手术前(术前)及心功能维护干预治疗后血浆中脑利钠肽(BNP)、肌钙蛋白Ⅰ(cTnⅠ)水平,探讨BNP及cTnⅠ与心力衰竭(心衰)的关系和CHD术前心衰的发生率,并评价心功能维护治疗的效果及意义。方法:选取2009年3月~2011年8月佛山市顺德妇幼保健院收治的50例小儿先天性心脏病为术前试验组患儿、50例无心衰支气管肺炎对照组患儿,检测心功能维护治疗前后的BNP、cTnⅠ水平并进行统计学比较。结果:试验组50例术前21例BNP、18例cTnⅠ异常升高,采用国际改良Ross标准诊断心衰20例(40.0%),与BNP、cTnⅠ指标符合率分别为92.6%、90.0%;对照组50例BNP无1例异常,2例cTnⅠ异常升高,两组比较差异有统计学意义(P<0.01)。试验组患儿经干预治疗后BNP、cTnⅠ指标比较差异有统计学意义(P<0.01)。结论:心衰干预治疗能显著改善心脏功能。

【Abstract】 Objective:To explore the relationship between brain natriuretic peptide(BNP),cardiac troponin Ⅰ(cTnⅠ) and heart failure,and the incidence of heart failure before surgery for the children with congenital heart disease(CHD) by detecting the plasma levels of BNP and cTnⅠ before surgery and after treatment of maintenance and intervention of heart function,evaluate the effect and significance of maintenance of heart function.Methods:Fifty children with CHD who were treated in the hospital from March 2009 to August 2011 were selected as experimental group,and fifty children with bronchopneumonia and without heart failure were selected as control group.The levels of plasma BNP and cTnⅠ before and after maintenance of heart function were detected and compared statistically.Results:In experimental group,21 children were found with abnormal increase of BNP level,18 children were found with abnormal increase of cTnⅠlevel,20 children(40.0%) were diagnosed as heart failure according to national improved Ross diagnostic criteria,the coincidence rates of BNP and cTnⅠwith Ross criteria were 92.6% and 90.0%,respectively.In control group,no child was found with abnormal BNP level,two children were found with abnormal increase of cTnⅠlevel,there was statistically significant difference between the two groups(P<0.01).In experimental group,there was statistically significant difference in BNP and cTnI levels before and after intervention(P<0.01).Conclusion:Intervention treatment can improve the cardiac function of the children with heart failure significantly.
