【摘要】 目的探讨新生儿肺炎克雷伯菌肺炎的药敏情况,及产超广谱β-内酰胺酶(ESBLs)的发生率,以指导临床合理用药。方法对我院75例确诊为新生儿肺炎患儿的痰液标本进行分离培养,采用长沙天地人微生物分析系统鉴定,同时用其相配套的药敏试验卡进行药敏试验,以及超广谱β-内酰胺酶(ESBLs)的检测,并对其检测结果进行统计分析。结果 75株肺炎克雷伯菌中检出产ESBLs菌株32株,阳性率为42.67%。不管何种类型的肺炎克雷伯菌对氨苄西林耐药率达100%,而对美洛培南敏感率达100%,对哌拉西林他唑巴坦的敏感率为74.67%。产ESBLs肺炎克雷伯菌的耐药率远远高于非产ESBLs的菌株(P<0.01或P<0.05)。结论新生儿肺炎克雷伯菌中产ESBLs菌株比例较高,耐药性强,对常用的抗生素不敏感。临床医师应根据药敏试验结果选用抗生素,以减少ESBLs菌株的产生。更多还原

【Abstract】 Objective To discussed the drug sensitivity of Neonatal pneumonia K.pneumoniae and the Incidence rate of ESBLs,so as to direct clinical rationally use drugs.Methods 75 cases of neonatal pneumonia K.pneumoniae for isolation and culture,isolated and identified with "TIANDIREN"microbial identification system,and the resistance sensitivity test was detected with test card corresponding to"TIANDIREN"microbial identification system,and the test of the ESBLs,statistical analysised the detecting result.Results There were 32 cases ESBLs was detected in 75 cases of K.pneumoniae,the positive rate was 42.67%,no matter What type of K.pneumoniae the drug resistance rate of Ampicillin was 100%,and the sensitivity rate of Meropenem was 100%,the sensitivity rate of Piperacillin tazobactam was 74.67%,the drug resistance rate in ESBLs K.pneumoniae was higher than the Non ESBLs K.pneumoniae(P<0.01 or P <0.05).Conclusion There was A higher proportion and strong drug resistance in ESBLs K.pneumoniae,and not sensitive for the commonly used antibiotics,the clinician would be according to the results of drug sensitivity test to selection of antibiotics,so as to reduction the produce of the ESBLs.更多还原
