冠心病合并单纯餐后高血糖的患病情况调查及其危险因素分析

【摘要】 目的了解聊城地区冠心病患者中单纯餐后高血糖的流行情况,探讨餐后2h血糖(2hPG)水平与冠心痛发病之间的关系,同时对冠心病合并单纯餐后高血糖患者的多种危险因素进行分析,以加深冠心病合并单纯餐后高血糖危险因素的认识,为其及早诊断、干预提供理论依据。方法选择400例冠心病病人和300例正常健康对照者,检测2hPG,血浆Hcy,C-反应蛋白水平,并对比分析。结果冠心病组2hPG明显高于健康组,两组比较差异有统计学意义(P﹤0.05)。冠心病组血浆Hcy水平高于对照组(P﹤0.05),差异有统计学意义,且血浆Hcy与血糖呈正相关。冠心病组C-反应蛋白水平高于对照组(P﹤0.05),有糖代谢异常组高于单纯冠心病组。结论冠心病患者糖代谢异常的发生率较高,餐后2h血糖可能是冠心病的独立危险因素,应加强糖调节受损人群的早期发现及干预,防止或延缓糖尿病大血管并发症的发生、发展,同时,Hcy,CRP与冠心病病发病密切相关,并且糖代谢异常可伴有Hcy、CRP增高。

【Abstract】 OBJECTIVE To know the popularity of the pure postprandial hyperglycemia among the patients with coronary heart disease in Liaocheng;explore the relationship between the level of 2 hPG and the attack of coronary heart disease;at the same time,analyze the risk factors of the coronary heart disease combined with the pure postprandial hyperglycemia to further deepen the recognition of the risk factors and provide theoretical basis for early detection and intervention.METHODS Selected 400 patients with coronary heart disease and 300 healthy people as control group.Made comparison study by inspecting the levels of 2 hPG,blood plasma Hcy and C-reactive protein.RESULTS The level of 2 hPG of the group with coronary heart disease was obviously higher than that of the control group.The comparative difference between two groups had statistical significance(P﹤ 0.05).The level of blood plasma Hcy was higher than that of the control group(P﹤0.05) and the difference was significant.Besides,the level of blood plasma Hcy had a positive relationship with the level of blood sugar.The level of C-reactive protein of the group with coronary heart disease was higher than that of the control group(P﹤0.05) and sugar metabolism of the abnormal group was higher than the group with pure coronary heart disease.CONCLUSION The occurrence of disorders of the glucose metabolism is high among patients with coronary heart disease.2hPG may be an independence risk factor.The early detection and intervention on the people with damaged glycoregulation should be strengthened,in this way to prevent or delay the occurrence and development of diabetes mellitus great vessels complication.At the same time,Hcy and CRP are closely related to coronary heart disease attack and the disorders of glucose metabolism always accompany with the increase of Hcy and CRP.
