低剂量多层CT对主支气管的动态评价
Low-dose dynamic CT assessment of the main bronchi
【摘要】 目的 探讨多层CT低剂量技术对主支气管动态扫描的优势、作用以及临床意义。方法 采用多排CT对34例健康成人行深吸气、深呼气末屏气扫描,覆盖整个中央气道,用120 kV,60 mA,5 mm层厚连续扫描,骨重建算法,1.25 mm连续薄层重建。以薄层图像为基础行三维立体重建,垂直于主支气管中轴做主支气管的轴位重建。全窗宽中点窗位法测量主支气管的各径线值。结果 深呼气末,左主支气管气道狭窄为(23.1±10.9)%(男)、(25.1±10.9)%(女)。右主支气管气道狭窄为(25.1±10.6)%(男)、(20.4±8.7)%(女)。吸气末主支气管呈圆形、卵圆形,呼气末呈后壁变平或轻度前弓形、甚至半月形。结论 低剂量动态CT是评价主支气管生理病理、结构量化改变的有效工具。
【Abstract】 Objective To study the advantages and clinical value of low-dose dynamic CT in evaluating the main bronchi.Methods Thirty-four healthy volunteers underwent chest low-dose dynamic CT examination with 120 kV and 60 mA,contiguous 5 mm thickness,bone algorithm.The scannings were performed at end-inspiration and end-expiration,and covered the entire central airway with MSC T.1.25 mm consecutive axial reconstructions were performed.Axial-reconstruction of the main bronchi were performed with images perpendicular to the bronchi axis.The diameters and cross-sectional areas of the bronchi were measured with full-width-half-maximum method.Results The mean cross-sectional area decreased by(23.1±10.9)%(male),(25.1±10.9)%(female) respectively of the left bronchi,and(25.1±10.6)%(male),(20.4±8.7)%(female) respectively of the right bronchi at end-expiration.The shapes of the bronchi were round or elliptical at end-inspiration,and posterior wall either flattens or bows slightly forward,even lunate shaped at end-expiration.Conclusion Low-dose dynamic CT can be used to evaluate the physiological and pathological changes of the main bronchi.
