小儿肺炎临床诊断及疗效评价指标专家问卷调查分析

【摘要】 目的:通过对小儿肺炎临床诊断及疗效评价指标临证经验的探讨,以期最终为科学、客观、系统的小儿肺炎临床诊断和疗效评价提供依据。方法:根据文献资料和预试结果,编制"小儿肺炎随机对照临床试验干预和对照措施及疗效评价指标"第一轮专家问卷,向51位儿科专家发放调查问卷并对其回信进行统计分析。结果 :在临床症状重要性评价中,咳嗽的秩均值最小;在引起高热不退因素的评价中,胸部大片影的秩均值最小;在临床诊断依据评价中,肺部罗音的秩均值最小;在出院及治愈标准评价中,发热消失的秩均值最小;在遗留问题评价中,慢性咳嗽的秩均值最小。结论:咳嗽是小儿肺炎急性期最常见的临床症状,胸部大片影是引起小儿肺炎高热不退的最主要因素,肺部罗音是小儿肺炎临床诊断的最主要依据,发热消失是临床上小儿肺炎出院及治愈的最重要的标准,慢性咳嗽是小儿肺炎最常见的遗留问题。

【Abstract】 Objective:To provide scientific,objective,systematic basis for diagnosis condition and curative effect determinant condition of pneumonia in children,it discussed the clinical experience of diagnosis condition and curative effect evaluation condition of pneumonia in children.Methods:According to documents and the information,it is compiled that the first turn of questionnaires of random controlled clinical trial and measurement and curative effect evaluation condition of pneumonia in children.They were distributed to 36 experts in this field.The questionnaires were analyzed with the method of statistics measurement.Results:The numerical value of coughing was minimal in common symptoms of clinical evaluation.The numerical value of chest radiograph was minimal in factors of high fever evaluation.The numerical value of lung auscultation was minimal in common diagnosis condition evaluation.The numerical value of fever gone was minimal in cure standard evaluation.The numerical value of chronic cough was minimal in questions left over by pneumonia evaluation.Conclusion:Coughing is the most common clinical symptom of the acute pneumonia in children;chest radiograph is the most important factor of high fever;lung auscultation is the main clinical diagnosis condition of pneumonia in children;fever gone is the most important cure standard of pneumonia in children;chronic cough is the most common question left over by pneumonia in children.
