【摘要】 目的探讨肾透明细胞癌与乏脂性肾血管平滑肌脂肪瘤的CT特征,提高诊断准确率。方法回顾性分析18例肾透明细胞癌与5例乏脂性肾血管平滑肌脂肪瘤的CT表现,所有病例均进行多层螺旋CT多期扫描。结果 18例肾透明细胞癌平扫表现为肾实质内均匀或不均匀的等、稍低或稍高混杂密度肿块,增强皮质期或实质期明显不均匀强化,强化程度等于或高于肾皮质,髓质期强化程度迅速下降,其中16例出现不同程度的囊变、坏死和/或出血,钙化1例。5例乏脂性肾血管平滑肌脂肪瘤平扫表现为肾实质内均匀的稍高密度肿块,增强后皮质期强化程度等于、低于或高于肾皮质,髓质期强化程度有所下降。5例均未见坏死、囊变、出血及钙化。结论肾透明细胞癌与乏脂性肾血管平滑肌脂肪瘤CT表现具有一定特征,术前大部分可准确诊断。更多还原

【Abstract】 Objective To discuss CT features of renal clear cell carcinoma and renal angiomyolipoma with less adipose tissue in order to improve its diagnostic accuracy.Methods Of the 23 patients,CT findings of renal clear cell carcinoma(18 patients) and renal angiomyolipoma were with less adipose tissue(5 patients) were analyzed retrospectively.All cases received multiple phase enhanced CT examinations.Results Renal clear cell carcinoma appeared as homogeneous or inhomogeneous masses,inhomogeneous density on plain CT images,inhomogeneous enhancement after administration of contrast media,and the most obviously enhanced portion of lesions was isodense or slight hyperdense relative to adjacent renal cortex in cortico phase or parenchymal phase.Hemorrhage,necrosis and cystic degeneration were seen in 16 lesions and calcification in 1 lesion.Renal angiomyolipoma with less adipose tissue appeared as slight hyperdense masses on plain CT images,inhomogeneous obvious enhancement after administration of contrast media,and the most obviously enhanced portion of lesions was iso,slight hypo or hyperdensity relative to adjacent renal cortex in cortico phase.Hemorrhage,necrosis and cystic degeneration and calcification were not seen.Conclusion Renal clear cell carcinoma and renal angiomyolipoma with less adipose tissue have different CT features,most of them can be diagnosed before surgical operation.更多还原
