【摘要】 目的:旨在探讨发病时D-二聚体(D)数值对脑梗死预后的影响,从而为脑梗死判定预后提供临床指导。方法:64例脑梗死患者入院时均由神经专科医师应用mRS及NIHSS量表进行评分,建立卒中登记。入院后第4天检查D-D数值,将其分成增高组(D1组)及正常组(D0组)。同时分析各个组之间与评分的相关性,对入选患者1个月及3个月后再次进行mRS、NIHSS评分,进行统计学分析,得出结论。结果:急性脑梗死发病第4天时D1组占64.1%,D-D值已明显升高,两组在mRS及NIHSS评分对比有统计学意义(P<0.01)。两组在发病1个月及3个月后,mRS及NIHSS评分均下降,D1组评分改善不及D0组,两组差异有统计学意义(P<0.01)。结论:我们选择发病后第4天D-D测定值作为研究指标。D-D的水平反映着体内高凝状态,D-D值直接影响患者发病时神经功能缺损的严重程度,影响着预后。更多还原

【Abstract】 Objective To explore the effect of D-D on the prognosis of cerebral infarction,which thereby can perovide clinical guidance to the prognosis assessment.Method Total 64 cases of cerebral infarction patients admitted to the hospital were classified and scored by neurologist according to the mRS and the NIHSS grading standard,and apoplexy registrations were established.Four days after the onset,they were divided into increasing groups(D1 group) and normal group(D0 group).Between the various groups,also analyzed the correlation with the score on selected patients after one month and three months the mRS,NIHSS score,was used for statistical analysis,concluded.Results Acute cerebral infarction on fourth day after the onset,D1 group accounted for 64.1%..D-D in D1 group increased significantly.mRS and NIHSS of the two groups differed significantly(P<0.01).One month and 3 months after the onset,mRS and NIHSS of the two groups dropped.D1 group increased less than D0 group.The differences between the two groups were significant(P<0.01).Conclusion We choose D-D value as indicators of research on the fourth day they are ill.D-D level reflects the physical hypercoagulable state.The D-D value directly impacts on patients′ neurological defect severity and affects the prognosis.更多还原
