加压氧对大鼠实验性肛门病术后创面的影响
THE INFLUENCE OF HYPERBARIC OXYGEN ON WOUND SURFACE AFTER SURGERY FOR DISEASE OF ANUS IN EXPERIMENTAL RATS
【摘要】 目的观察加压氧对大鼠实验性肛门病术后创面组织水肿及修复的影响,并探讨其作用机制。方法选取80只Wistar大鼠,采用角叉菜胶注射致大鼠肛周肿胀、醋酸致肛周溃疡建立实验性肛门病术后模型,并随机分为中草药熏治组、常压氧治疗组、加压氧治疗组和空白对照组,每组20只。术后第2天起,中草药熏治组予中草药熏洗,每天2次,每次15min;常压氧治疗组予体积分数0.85氧常压下暴露,每天2次,每次60min;加压氧治疗组予体积分数0.85氧加压暴露,每天2次,每次60min;空白对照组建模后不做任何处理,实验时间7d。分别于建模后第1、2、3、5、7天观察创面的修复情况及组织消肿情况,并于第8天处死大鼠,按WEIDNER法计算肛周水肿及溃疡组织中CD105标记的组织微血管密度(MVD)。结果各治疗组大鼠肛周水肿均较空白对照组减轻,治疗3、5、7d加压氧治疗组与中草药熏治组和常压氧治疗组比较,差异亦有显著性(F=10.33~28.58,q=4.30~6.44,P<0.05)。从治疗3d开始,加压氧治疗组大鼠肛门溃疡面积小于空白对照组,差异有显著性(F=3.24~3.64、q=3.98~4.66,P<0.05);治疗后5、7d,加压氧治疗组与其他治疗组比较差异有显著性(q=4.01~5.93,P<0.05)。加压氧治疗组MVD高于其他各组,差异有显著性(F=6.71~9.02,P<0.05)。结论加压氧可改善创面局部微循环,促进组织消肿,提高创面愈合质量。
【Abstract】 Objective To observe the effect of hyperbaric oxygen(HBO) on tissue edema and repair of wound surface after surgery for disease of anus in experimental rats,and study its mechanism of action.Methods A postoperative rat model of experimental disease of anus was created in 80 rats by injecting carrageenan to cause perianal swelling and ulcer,the rats wereequally randomized to four groups as Chinese herbal-medicine-fumigation(CHMF),normobaric-oxygen(NBO),HBO,and blank control.Starting from the second day after surgery,fumigation and wash with Chinese herbal medicine was offered to the wounds of the rats in CHMF group,two times a day,15 min for each time;the wounds of those in NBO group were exposed to oxygen concentration of 85%,twice a day,60 min for each exposure;those in HBO group,pressure exposure to oxygen concentration of 85%,twice a day,60 min each time;to those in blank control group,no any treatments were given.The experiment lasted for se-ven days.The healing and subsiding swelling of tissue of the wounds were observed at days 1,2,3,5 and 7 upon the completion of the model,the rats were killed at day 8,the microvessel density(MVD) in edema and ulcer tissues labeled by CD105 was calculated according to WEIDNER method.Results In the three treatment groups,the perianal edema was lightened than the blank control.The differences among the three treatment groups(at days 3,5 and 7 of therapy) were significant(F=10.33-28.58,q=4.30-6.44,P<0.05).Starting from the third day of therapy,the area of anal ulcer in rats of HBO group was smaller than that of the control,with a significant difference between them(F=3.24-3.64,q=3.98-4.66,P<0.05);at days 5 and 7,the differences between HBO and the other two treatment groups were significant(F=3.24-5.63,q=4.01-5.93,P<0.05).The expression of MVD in HBO group was higher than that in the other groups,the differences among them being significant(F=6.71-9.02,P<0.05).Conclusion Hyperbaric oxygen can improve the local micro-circulation of wound surface,promote swelling of tissue,and raise the quality of wound healing.
