复发性阴道假丝酵母菌病的病原菌鉴定及药物敏感性分析

【摘要】 目的探讨复发性阴道假丝酵母菌病(RVVC)的致病菌种,并对其药物敏感性进行分析。方法将138例RVVC和外阴阴道假丝酵母菌病(VVC)患者的阴道分泌物标本,按常规方法处理后,接种于萨布罗琼脂平板培养基上,在37℃恒温箱中培养2~3d,纯化后采用法国科玛嘉显色培养基、微生物全自动鉴定系统VITEK-2鉴定,采用微量液体培养基稀释法,对分离的假丝酵母菌属进行氟康唑、咪康唑、酮康唑、克霉唑、伊曲康唑、制霉菌素及特比萘芬敏感性试验。结果 RVVC组分离出假丝酵母菌属79株,VVC组分离出假丝酵母菌属79株;2组分离出菌株均以白色假丝酵母菌为主,各占68.35%和78.48%,RVVC组白色假丝酵母菌株与VVC组比较明显减少,差异有统计学意义(P<0.05),光滑假丝酵母菌株占20.25%,与VVC组比较显著增多,差异有统计学意义(P<0.05);RVVC组对制霉菌素敏感率为93.67%,VVC组为96.20%。结论 RVVC的主要致病菌为白色假丝酵母菌,但RVVC组非白色假丝酵母菌比例高于VVC组,非白色假丝酵母菌中以光滑假丝酵母菌比例最高,药敏试验2组均对制霉菌素较敏感。

【Abstract】 OBJECTIVE To evaluate the pathogenic species for recurrent vulvovaginal candidiasis(RVVC) and analyze their drug sensitivity. METHODS The vaginal secretion samples of 138 cases RVVC and VVC patients were dealt by conventional treatment and then inoculated on sabouraud agar medium to cultivate them 2-3days at 37 ℃ incubator.The French Kema Jia color medium,microbial identification system,VITEK Ⅱ automatic were used for identification after they were purified.According to Micro-dilution method,using liquid medium,a susceptibility test was planed for the separation of Candida to fluconazole,miconazole,ketoconazole,clotrimazole,itraconazole,nystatin,and terbinafine. RESULTS In RVVC group,79 Candida were isolated 79 Candida isolated from VVC group.They were mainly Jurassic each 68.35% and 78.48% for two isolated Candida albicans strains of bacteria.RVVC strains of Candida albicans group was less than the VVC significantly,and the difference was significant(P<0.05).Candida glabrata strains accounted for 20.25%,compared with VVC,it was significantly increased,the difference was significant(P<0.05).Susceptibility testing of both groups were more sensitive to nystatin,respectively,93.67%,96.20%. CONCLUSION Candida albicans is the major pathogen for RVVC,but the rate of RVVC group of non-albicans VVC group is higher than the proportion of non-Candida albicans.Candida glabrata is the highest proportion.Susceptibility testing of both groups are more sensitive to nystatin.
