乌司他丁对小型猪严重烧伤后心肌细胞的保护作用

【摘要】 目的观察乌司他丁(UTI)对小型猪严重烧伤后炎性介质、氧自由基释放及心肌酶谱的影响。方法将贵州三系雄性小型猪12只随机分为A组(烧伤对照组,n=6)和B组(UTI治疗组,n=6)。所有动物均造成35%TBSAⅢ度烧伤,其中A、B两组动物于烧伤前各随机选取4只抽取静脉血作为正常对照。B组动物于伤后1h给予UTI5000U/kg,A组动物给予等量生理盐水,3次/d。分别于动物烧伤前及伤后6、24、48、72h抽血进行血清肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、丙二醛(MDA)、超氧化物歧化酶(SOD)、肌酸激酶(CK)、肌酸激酶同工酶(CKMB)、乳酸脱氢酶(LDH)含量检测。结果严重烧伤后动物血清TNF-α、IL-6、MDA、CK、CKMB、LDH含量明显升高,SOD显著减少。以A组尤为明显,与B组比较差异有统计学意义(P<0.05);A组血清CK、CKMB、LDH含量于各时间点均较B组显著升高(P<0.05)。结论严重烧伤可造成大量炎性介质释放和氧自由基产生,从而导致心肌损害,乌司他丁能显著抑制炎性介质及中性粒细胞蛋白酶的释放和氧自由基产生,减轻烧伤后心肌损害程度。

【Abstract】 Objective To investigate the efficacy of ulinastatin(UTI),a human trysin inhibitor,on the inflammatory mediators and oxygen free radical and protection of cardiac myocyte at the early stage of severe burn.MethodsTwelve Guizhou male mini-pigs were randomly divided into group A(control group,n=6) and group B(UTI-treated group,n=6),causing 35% third-degree burn injury,and fluid resuscitation was received immediately after injury.UTI(5000 U/kg,one hour after injury,3 times a day) and an equivalent physiological saline were administered to groups B and A,respectively.Blood samples were taken for the determination of serum tumor necrosis factor-alpha(TNF-α),interleukin-6(IL-6),malonyldiadehyde(MDA),superoxide dismutase(SOD),creatine kinase(CK),MB isoenzyme of CK(CKMB) before injury and 6,24,48,72 hours after injury.ResultsA marked elevation in serum TNF-α,IL-6,MDA,CK,CKMB and a significant reduction in serum SOD were found after severe burn injury,especially in group A.In group B,the increase in serum TNF-α,IL-6,MDA and decrease in SOD were distinctly sup pressed(P< 0.05).The serum content of CK and CKMB in group A were always higher than those in group B at any time point evaluated(P<0.05).ConclusionsThe results indicate that severe burns can cause the release of inflammatory mediators and oxygen free radical and cardiac myocyte injury.UTI can significantly lessen the production of inflammatory mediators and oxygen free radical and protect cardiac myocyte after severe burn.
