【摘要】 目的:探讨应用不同方式的宫腔镜手术治疗子宫内膜息肉的临床治疗效果。方法:根据121例子宫内膜息肉病人的年龄、有无生育要求而分别实行不同的宫腔镜手术,A组57例,育龄期妇女有生育要求者,切除息肉及息肉旁浅层内膜(切除内膜的功能层);B组41例,育龄期妇女无生育要求者,切除息肉的同时行子宫内膜电切术(即切除子宫内膜的功能层及基底层);C组23例,绝经患者行息肉切除同时行子宫内膜剥除术(滚球电极及电凝破坏内膜功能层和基底层)。结果:86例月经紊乱者月经失血图评分术前平均(314±145)分,术后平均(35±20)分,手术前后月经量比较,差异有统计学意义(P<0.001)。A组术后复发8例(14.03%,8/57),B组术后复发1例(2.44%,1/41),C组无复发。A与B、C两组相比,差异有统计学意义(P<0.05)。121例手术均未发生严重的并发症。结论:宫腔镜下不同手术方式治疗子宫内膜息肉临床效果无明显差异,育龄期妇女有生育要求切除息肉及息肉旁浅层内膜者的术后复发率较高(14.03%,6/57)。应根据不同年龄、不同生育要求选择相适宜的宫腔镜手术方式治疗子宫内膜息肉。更多还原

【Abstract】 Objective:To Investigate the therapeutic efficacy of different types of hysteroscopic surgery for endometrial polyps. Methods:A total of 121 cases were operated by different ways of hysteroscopic surgery for endometrial polyps,according to ages and the desire of childbearing of the patients.57 cases of group A who desired future childbearing,did polypectomy with endometrial resection of superficial layer near the polyps.41 cases of group B did polypectomy with endometrial resection.23 postmenopausal patients of group C did polypectomy with endometrial coagulation. Results:By the pictorial blood loss assessment chart(PBAC) there was significant difference between before and after the surgery(P<0.001).None of the polyps recurred for the patients of group C,but the recurrent rate of group A was 14.03%,and the group B was 2.44%,the recurrent rate of groups A was higher than other tow groups(P<0.05).there was no major complication for 121 patients. Conclusion:It is feasible to select different hysteroscopic surgery for endometrial polyps,according to ages and the desire of childbearing of the patients.更多还原
