双源64层螺旋CT在冠状动脉粥样硬化性心脏病诊断中的价值

【摘要】 目的评价双源64层螺旋CT冠状动脉成像在诊断冠状动脉粥样硬化性心脏病中的价值。方法 110例冠心病临床诊断或可疑病例在1周内先后行双源64层螺旋CT冠状动脉成像及冠状动脉造影检查,对左右冠状动脉及主要分支的狭窄程度进行诊断,以冠状动脉造影的结果作为诊断标准,评价双源64层螺旋CT诊断冠心病的敏感性、特异性、阳性预测值、阴性预测值及符合率。结果双源64层螺旋CT冠状动脉成像质量高,可评价的血管比例在90%以上,与冠脉造影结果比较的敏感性为77.4%、特异性为94.1%、阳性预测值为74.9%、阴性预测值为94.9%、符合率为91.8%。结论双源64层螺旋CT冠状动脉成像对冠状动脉狭窄的诊断具有较高的准确行,可作为高危人群筛选的首选方法。

【Abstract】 Objective To evaluate the diagnostic significance of dual-source 64-slice spiral computed tomogra-phy in the diagnosis of coronary heart disease.Methods 110 patients with suspected coronary artery disease underwent coronary angiography and dual-source 64-slice spiral computed tomography.The mean time span between coronary angiography and dualsource 64-slice spiral CT was 7 days.To diagnose the stenosis of left main,left ante-rior descending artery,circumflex branch,and right coronary artery.Invasive coronary angiography was taken as gold standard for calculations of diagnostic accuracy.To evaluate the dual-source 64-slice spiral computed tomography’s sensitivity,specificity,positive predictive value,negative prdictive value and accuracy in the diagnosis of coronary heart disease.Results Dual-source 64-slice spiral CT coronary angiography was of high quality,the proportion of bloodvessels could be evaluated more than 90%.Compared with coronary angiograpy,the sensitivity of CT to diagnose significant stenosis was 77.4%,specificity 94.1%,positive predictive value 74.9%,negative predictive value 94.9% andaccuracy 91.8%.Conclusion Dual-source 64-slice spiral computed tomography has a high accuracy for the detection of significant coronary artery stenoses and therefore can be considered as a valuable noninvasive technique.
