【摘要】 目的分析上海市公立医疗机构收入结构及变化趋势,为公立医院补偿机制改革提供数据支持和政策建议。方法资料来源于2005-2009年上海市8所综合医院和20所二级中心医院以及上海市16个区的社区卫生服务机构财务收支数据。分三级医疗机构建立Excel 2003数据库,进行公立医院收入结构和趋势分析。结果政府投入逐年增加,但维护"公益性"的基石仍然薄弱;医疗补偿比例较低,未能反映医疗服务价值;医院药品补偿畸形,"以药养医"亟待破局。结论我国公立医疗机构收入结构不尽合理,需进一步完善。更多还原

【Abstract】 Objective To analyze the economic revenue structure and the development trend at multiple levels of public hospitals in Shanghai,and to provide data and policy recommendations for the reform of public hospital revenue compensation mechanism. Methods Related data came from the balance sheets of multi-level public hospitals in Shanghai between 2005 and 2009,which constituted 8 comprehensive hospitals,20 second-level central hospitals and community health services of 16 administrative districts. Excel 2003 database was established on the basis of a 3-level health service structure.And then,independent analyses on the economic revenue structure and its development trend of public hospitals were made within the three-level hospitals in Shanghai.Results Though the input from the city government increased each year,the cornerstone of protecting public welfare was still weak.Compensation rate of health service remained at a relatively low level,which failed to reflect the actual value of health service.Drug compensation of hospitals was lopsided,for this reason,"the policy of supporting the hospital by selling drugs"needed rectified urgently.Conclusion The revenue structure of public hospitals in our country was not reasonable and needed further improvement.更多还原
