【摘要】 目的:观察急性心肌梗死康复期患者对6个月心脏康复运动的依从性及运动疗效,患者康复运动依从性与家庭关怀度、心理状况的关系。方法:首次发作急性心肌梗死并无任何并发症的患者,出院后可参加心脏康复运动。护士在患者出院前根据其运动试验结果,给他们制定相应的个体化运动处方,嘱患者出院后6个月按照各自的处方坚持康复运动,护士要告知患者运动的注意事项。在此期间不予患者特殊干预措施,但患者可来电咨询相关问题。当患者出院6个月后对其回访时,按照达到每周建议运动总时间的80%为患者依从性好的标准,采取方便取样,将对康复运动依从性好的28例患者设为实验组;将依从性不好(没做运动或偶尔做一两次运动,运动不规律)的31例患者设为对照组。对全部患者(59例)采取的指标评价为无氧阈(VO2AT),使用的评价工具为家庭关怀度指数问卷(家庭APGAR量表)、自评抑郁量表(SDS),并进行logistic回归分析。结果:实验组较对照组而言无氧阈有显著提高,运动能力及运动耐力明显提高;实验组家庭关怀度得分(8.36±1.68)显著高于对照组(5.87±1.71),SDS自评抑郁得分(30.82±5.24)显著低于对照组(41.55±3.98)。结论:较好的康复运动依从性有助于提高患者运动能力以及运动耐力;家庭的关怀有助于提高患者康复运动依从性;负性的情绪对康复运动依从性将产生负面影响。更多还原

【Abstract】 Objective: To observe AMI patients’ rehabilitation compliance and exercise effects, and to explore the relationship between family care, mental status and rehabilitation compliance. Methods: Patients were provided appropriate individualized exercise prescription depending on their test results before discharge after first episode of acute myocardial infarction. After six months, 28 patients who achieved 80% of the recommended exercise time per week were chosen as the experimental group, while 31 patients with poor rehabilitation compliance were chosen as the control group by convenient sampling. Both were evaluated the anaerobic threshold (VO2AT), surveyed using the family care index questionnaire (Family APGAR Scale) and the self-rating depression scale (SDS). Results: Patients in the experimental group showed significantly higher anaerobic threshold (VO2AT), exercise capacity and endurance, and got significantly higher family care scores, lower SDS depression scores compared with those in the control group. Conclusion: Good rehabilitation compliance improves AMI patients’ exercise capacity and exercise endurance, home care helps to improve their rehabilitation compliance; negative mental status plays a negative impact on the rehabilitation compliance.更多还原
