64层CT重建技术在胫骨平台骨折诊疗中的应用价值

【摘要】 目的探讨64层CT多种重建技术在胫骨平台骨折的诊断与治疗中的应用价值。方法收集经手术治疗的33例胫骨平台骨折患者的临床及影像学资料,所有患者术前均经X线平片检查、64层CT检查后多平面重建(MPR)和三维(3D)重建。回顾性分析33例患者的CT检查及重建图像,进行骨折分型,评估胫骨平台的劈裂及塌陷程度,并与X线平片和手术结果对照。结果 33例胫骨平台骨折的CT图像经MPR及3D重建后,骨折显示与术中所见吻合率为100%,X线平片漏诊1例。MPR能够显示骨折的细微结构,有助于骨折类型的判断。3D重建可清晰显示骨折的立体外观、关节面碎裂及塌陷的范围。结论 64层CT重建技术能直观、多方位、立体地显示胫骨平台骨折的部位、形态、类型,关节面碎裂及塌陷程度等空间信息,对选择适当的手术方式、准确的复位和内固定具有重要的参考价值。

【Abstract】 Objective To evaluate the value of MSCT reconstruction in diagnosis and treatment of tibial plateau fractures. Methods The clinical and imaging data from 33 patients with tibial plateau fracture were collected and retrospectively reviewed. All patients were examined by standard X-rays and MSCT with MPR and 3D reconstruction. the fractures were classified and the degree of splitting and depression were evaluated. Finally, compared with X-ray and operation findings. Results After MPR and 3D reconstruction with MSCT, all 33 patients of tibial plateau were absolutely accorded with operation findings, 1 case was omitted by X-ray. MPR is able to show interior fine situation of fractures and helpful for the classification of fractures. 3D reconstruction can clearly display stereo anatomy of fracture, broken articular surface and scope of collapse of the plateau. Conclusion MSCT reconstruc tion techniques can provide an advantage of showing the location,shape, patten, and degree of tibial plateau fractures more directly, stereoscopically and dimensionally. These techniques have an important value for surgeons to choose therapeutic, reduction and fixed method and surgical approach.
