非产超广谱β-内酰胺酶肺炎克雷伯菌耐药性分析
Resistance Analysis of Non-ESBLs-producing Klebsiella pneumoniae
【摘要】 目的了解非产超广谱β-内酰胺酶(ESBLs)肺炎克雷伯菌的耐药情况,为临床合理有效使用抗菌药物提供科学依据。方法回顾性地分析医院2006-2009年住院患者临床送检标本经分离确证的126株非产ESBLs肺炎克雷伯菌,用K-B法测定非产ESBLs肺炎克雷伯菌对12种抗菌药物的耐药率。结果确证的非产ESBLs肺炎克雷伯菌126株对氨苄西林、头孢唑林、氨苄西林/舒巴坦的耐药率>70.00%,对头孢呋辛、氧氟沙星、庆大霉素、环丙沙星、左氧氟沙星的耐药率>25.00%,对头孢噻肟、头孢曲松、阿米卡星<15.00%,对头孢哌酮/舒巴坦高度敏感,耐药率为2.38%。结论非产ESBLs肺炎克雷伯菌对青霉素类、第一、二代头孢菌素耐药严重,对喹诺酮类、氨基糖苷也存在不同程度的耐药,第三代头孢菌素和阿米卡星是临床治疗非产ESBLs肺炎克雷伯菌感染的有效药物。
【Abstract】 OBJECTIVE To understand the resistance circumstances of non-ESBLs-producing Klebsiella pneumoniae,to provide scientific bases for clinical reasonable and effective use of antibiotics.METHODS A total of 126 strains of non-ESBLs-producing K.pneumoniae isolated from hospital inpatients between 2006 and 2009 were retrospectively analyzed,the resistance rates of non-ESBLs-producing K.pneumoniae to 12 antibiotics were determined by K-B method.RESULTS The resistance rates of the confirmed 126 strains of non-ESBLs-producing K.pneumoniae to ampicillin,cefazolin and ampicillin/sulbactam were >70%,to cefuroxime,ofloxacin,gentamicin,ciprofloxacin and levofloxacin were >25%,and to cefotaxime,ceftriaxone and amikacin were <15%,non-ESBLs-producing K.pneumoniae was highly sensitive to cefoperazone/sulbactam,with the resistance rate 2.38%.CONCLUSION The drug resistance of non-ESBLs-producing K.pneumoniae to penicillins and the first and second-generation cephalosporin is severe,there are various degrees of resistance to quinolones and aminoglycoside.The third generation cephalosporins and amikacin are the effective drugs to treat the infection caused by non-ESBLs-producing K.pneumoniae.
