环氧合酶2和过氧化物酶体增殖因子活化受体γ在食管癌变过程中的表达变化及其关系

【摘要】 目的观察环氧合酶2(cyclooxygenase-2,COX-2)及过氧化物酶体增殖因子活化受体γ(peroxisomeproliferator-activated receptors,PPARγ)在不同食管病变组织中的表达,探讨二者在食管癌变过程中表达的意义。方法应用免疫组织化学方法,检测18例食管正常鳞状上皮、24例鳞状上皮异型增生及59例食管鳞状细胞癌中COX-2及PPARγ的表达,分析其与食管癌发生及临床病理特征的关系。结果 COX-2在食管正常鳞状上皮、鳞状上皮异型增生和鳞状细胞癌的阳性表达率分别为22.2%、29.2%和55.9%,鳞状细胞癌的COX-2阳性表达率高于正常鳞状上皮和鳞状上皮异型增生,差异有统计学意义(P<0.05),而且高分化鳞癌(68.4%)及中分化鳞癌(64.0%)的COX-2阳性表达率显著高于低分化鳞癌(26.7%)(P<0.05);相反,食管鳞癌的PPARγ的阳性表达率为52.5%,显著低于食管正常鳞状上皮(88.9%)和鳞状上皮异型增生(83.3%)(P<0.05);高分化、中分化及低分化鳞癌的PPARγ阳性表达率分别为89.5%、48.0%和13.3%,PPARγ的阳性表达率随食管癌分化程度的降低而降低,3组间的两两比较差异有统计学意义(P<0.05)。食管鳞癌中有淋巴结转移者COX-2表达低(P<0.05),但PPARγ表达与临床病理特征无关。在食管癌变过程,COX-2与PPARγ表达呈负相关(rs=-1.000,P<0.05),而在食管鳞状细胞癌中,COX-2与PPARγ表达呈正相关(rs=1.000,P<0.05)。结论在食管癌变过程中,COX-2表达显著增高,而PPARγ表达则显著降低,二者可能有相互制约作用。但在食管鳞状细胞癌中,COX-2及PPARγ表达均随分化程度降低而降低,提示二者在食管癌变及发展展过程中发挥不同的作用。

【Abstract】 Objective To investigate the expression and clinical significance of cyclooxygenase-2(COX-2) and peroxisome proliferator-activated receptors(PPARγ) in various lesions of esophageal tissues,the correlation between the expression of COX-2 and PPARγ was also analyzed.Methods The expressions of COX-2 and PPARγ in 18 specimens of normal squamous epithelium,24 specimens of squamous dysplasia and 59 specimens of esophageal squamous cell carcinoma(ESCC) were examined by SP immunohistochemistry,the expressions of COX-2 and PPARγ in the process of esophageal carcinogenesis were observed and their possible correlations with clinicopathologic features of ESCC were analyzed.Results The positive expression rate of COX-2 was 22.2% in normal squamous epithelium,29.2% in squamous dysplasia and 55.9% in ESCC.The expression of COX-2 was significantly higher in ESCC than in normal squamous epithelium and squamous dysplasia(P<0.05).In ESCC,the positive expression rate of COX-2 was higher in well differentiated ESCC(68.4%) and moderately differentiated ESCC(64.0%) than that of poorly differentiated ESCC(26.7%)(P<0.05).The positive expression rates of PPARγ in normal squamous epithelium,squamous dysplasia,and ESCC were 88.9%,83.3% and 52.5%,respectively.There was a significant lower expression of PPARγ in ESCC(ESCC vs normal squamous epithelium and squamous dysplasia,P<0.05,respectively).Among ESCC cases,the positive expression rate of PPARγ was closely related to the degree of differentiation,the positive expression rates of PPARγ decreased as the pathological grade increased(P<0.05).The expression of COX-2 was significantly lower in the samples from the patients with lymph node metastasis than in those without nodal metastasis(P<0.05),but there was no significant relationship between the positive expression rate of PPARγ and their clinicopathological characteristics.COX-2 expression was negatively correlated with PPARγ expression in the carcinogenesis of ESCC(rs=-1.000,P<0.05),but in ESCC,there was a positive correlation between the expression of COX-2 and PPARγ(rs=1.000,P<0.05).Conclusion In the carcinogenesis of esophagus,the positive expression rate of COX-2 increased,while PPARγ decreased,in ESCC,as the pathological grade of malignancy increased,the positive expression rate of COX-2 and PPARγ decreased.COX-2 and PPARγ are involved and may play different role in the carcinogenesis and progression of ESCC.
