HIV感染者与健康人群口腔念珠菌的体外药物敏感性比较

【摘要】 目的探讨HIV感染者与健康人群口腔念珠菌对常用抗真菌药物敏感性的异同。方法采用美国临床和实验室标准化研究所推荐方案M27-A2,即微量液基稀释法对60株HIV感染者口腔念珠菌和61株健康人群口腔念珠菌进行体外抗真菌药物敏感性试验,分别测定两种来源的念珠菌对氟康唑(fluconazole,FCZ)、酮康唑(ketoconazole,KETO)、氟胞嘧啶(fluorocytosine,5-FC)和两性霉素B(amphotericin B,Amb)共4种常用抗真菌药物的敏感性,SPSS 13.0统计分析。结果HIV感染组对上述4种药物敏感性的几何均数分别为0.87μg/mL、0.05μg/mL、0.18μg/mL和0.33μg/mL,耐药率分别为1.7%、5%、3.3%和5%,1株光滑念株菌对FCZ、KETO和Amb同时耐药;健康人群组敏感性的几何均数分别为3.45μg/mL、0.06μg/mL、0.30μg/mL和0.42μg/mL,耐药率分别为9.8%、6.6%、9.8%和9.8%,1株光滑念株菌对FCZ、KETO和5-FC同时耐药,另1株光滑念珠菌对KETO、5-FC和Amb同时耐药。结论除FCZ外,两组念珠菌对KETO、5-FC和Amb的敏感性差异无显著性(P>0.05);两组念珠菌对上述4种药物的耐药率差异无显著性(P>0.05);两组念珠菌中非白色念珠菌的耐药率均高于白色念珠菌(P<0.05),且存在交叉耐药现象。

【Abstract】 To investigate the difference in the in vitro anti-fungal susceptibility to oral species of candida isolated from HIV-infected patients and healthy subjects,the anti-fungal susceptibility test in 60 and 61 species of candida isolated from mouth cavity of HIV-infected patients and healthy subjects was performed to detect the sensitivity of the isolates to 4 anti-fungal drugs.i.e.fluconazole(FCZ),ketoconazole(KETO),flurocytosine(5-FC) and amphotericin B(Amb) by using broth micro-dilution test according to the guidelines of the Clinical and Laboratory Standard Institute(CLSI).The data obtained was analyzed by SPSS13.0 software.It was found that the geometric means of the Candida species to the above mentioned anti-fungal drugs were 0.87 μg/mL,0.05 μg/mL,0.18 μg/mL and 0.33 μg/mL respectively in HIV-infected patients,and the drug resistance rates of isolates were 1.7%,5.0%.3.3% and 5.0%,respectively.Whereas the geometric means in the healthy subjects of the candida species to the these 4 drugs were 3.45 μg/mL.0.06 μg/mL,0.3 μg/mL and 0.42 μg/mL respectively and the resistance rates of isolates were 9.8%,6.6%,9.8% and 9.8% respectively.One isolate of C.glabrata was resistant to FCZ,KETO and 5-FC and another isolate was resistant to KETO,5-FC and Amb simultaneously.It is evident that no significant difference exists in the susceptibility of candida species to KETO.5-FC and Amb except FCZ(P>0.05)in both groups and the resistance rate of C.non-albican species is rather higher than C.albicans in both groups(P<0.05) indicating the presence of cross-resistance phenomenon.
