正常中脑导水管与鞍区水平脑脊液流动对比研究
Comparative study between normal volunteers in aqueduct and sellar region of cerebrospinal fluid flow
【摘要】 目的应用磁共振相位对比法观察正常中脑导水管及鞍区脑脊液的流动情况,并进行对比分析。方法筛选32例正常志愿者,分别测量其中脑导水管及鞍区脑脊液流动情况,在正中矢状位T1WI或T2WI图像上选择鞍区及中脑导水管层面,编码速率20cm/s,计算鞍区水平脑脊液流动方向、流速、流量率等情况,并研究其与心动周期的关系。结果正常志愿者中脑导水管及鞍区脑脊液流动在一个心动周期内表现为双向流动,即收缩期向下流动、舒张期向上流动,收缩期向下最大流速(对应心动周期)、舒张期向上最大流速(对应心动周期)、平均向下流速、平均向上流速、平均流速在中脑导水管分别为(5.45±1.64)cm/s[(49.33±40.59)ms]、(5.35±1.91)cm/s[(505.33±106.27)ms]、(3.16±1.12)cm/s、(3.14±0.95)cm/s、(3.12±0.91)cm/s,在鞍区分别为(1.46±0.98)cm/s[(304.69±246.17)ms]、(1.18±0.62)cm/s[(332.12±224.52)ms]、(0.48±0.42)cm/s、(0.69±0.46)cm/s、(0.56±0.32)cm/s;收缩期向下最大流量率、舒张期向上最大流量率、平均向下流量率、平均向上流量率、平均流量率、平均净流量率,在中脑导水管分别为(0.053±0.017)ml/s、(0.053±0.019)ml/s、(0.030±0.011)ml/s、(0.031±0.010)ml/s、(0.031±0.010)ml/s、(0.045±0.034)ml/s,在鞍区分别为(0.016±0.010)ml/s、(0.013±0.008)ml/s、(0.044±0.046)ml/s、(0.055±0.003)ml/s、(0.055±0.003)ml/s、(0.036±0.032)ml/s;平均心动周期在中脑导水管为(717.97±89.79)ms,在鞍区为(777.36±175.12)ms。各项测量指标间差异有统计学意义(均P<0.05)。结论在脑脊液循环路径中,中脑导水管与鞍区脑脊液流动的参数及时间-速度曲线是不同的。
【Abstract】 Objective To quantitatively study the features of cerebrospinal fluid flow dynamics between normal aqueduct and sellar region of cerebrospinal of MRI.Methods Thirty-two normal volunteers were studied in aqueduct and sellar region respectively.The cerebrospinal fluid flow image was positioned sellar region and aqueduct at the middle sagittal T1WI or T2WI image.This pulse sequence used a velocity encoding of 20 cm/s.The waveforms were analyzed for the maximum flow velocity,flow volumerate of the figures.From the velocity and area measure means on the cine images,mean CSF flow were calculated in millimeters per second and milliliters per cardiac cycle.Results The CSF flow of the sellar region and aqueduct has two directions which is downward(caudal)flow during the systolic period and upward(cranial)flow during the diastolic period of the cardiac cycle.The out and in flow peak velocity and mean out and in flow and mean flow were(5.45±1.64)cm/s[(49.33±40.59)ms],(5.35±1.91)cm/s[(505.33±106.27)ms],(3.16±1.12)cm/s,(3.14±0.95)cm/s,(3.12±0.91)cm/s and(1.46±0.98)cm/s[(304.69±246.17)ms],(1.18±0.62)cm/s[(332.12±224.52)ms],(0.48±0.42)cm/s,(0.69±0.46)cm/s,(0.56±0.32)cm/s respectively.The out and in volumerate peak velocity and mean out and in volumerate and mean volumerate were(0.053±0.017)ml/s,(0.053±0.019)ml/s,(0.030±0.011)ml/s,(0.031±0.010)ml/s,(0.031±0.010)ml/s,(0.045±0.034)ml/s and(0.016±0.010)ml/s,(0.013±0.008)ml/s,(0.044±0.046)ml/s,(0.055±0.003)ml/s,(0.055±0.003)ml/s,(0.036±0.032)ml/s respectively.The mean cycle was(717.97±89.79)ms and(777.36±175.12)ms.The distances were prominenced discrepancy between Empty Sella Syndrome and normal volunteers(P<0.05).Conclusions In the cerebrospinal fluid circulation path and curve of time-velocity cerebrospinal fluid parameters of the aqueduct and the sellar region are different.
