【摘要】 目的:探讨头颅CT未显示脑白质变性的轻度认知损害(MCI)患者的事件相关电位(ERP)P300、脑电图(EEG)和血浆载脂蛋白B的特点,为早期血管性痴呆的诊断提供帮助。方法:对60例MCI患者行头颅CT检查,根据有无脑室周围、半卵圆区、皮质下白质斑片状影,将上述患者分为脑白质变性组及非脑白质变性组,均进行P300、EEG及血浆载脂蛋白B检查,进行相关性分析,并与健康老年组32例做比较。结果:脑白质变性组与非脑白质变性组P300检查比较,P300峰潜伏期有显著延长,部分非脑白质变性患者P300峰潜伏期有轻度延长,与健康组相比有显著差异;脑电图检查,脑白质变性组以δ、θ波为主,轻-中度异常比例显著高于非脑白质变性组,非脑白质变性组以α、β波为主;健康老年组以α波为主;血浆载脂蛋白B化验,各组之间无显著差异。结论:P300潜伏期延长提示MCI患者可能有早期血管性痴呆;EEG颞部θ波增多提示MCI患者可能有早期血管性痴呆。载脂蛋白B化验未见明显差异,在本次研究中对于早期血管性痴呆诊断无确切临床意义。更多还原

【Abstract】 Objective:To explore the characteristics of P300,electroencephalography(EEG) and apoprotein B in elderly pa tients with mild cognitive impairment(MCI) ,who had not myelinopathy in CT. Methods:The subjects were 60 cases with MCI. They were divided into 2 groups according to whether or not myelinopathy,and compare with 32 healthy elderly as control. The P300,EEG and apoprotein B were recorded from each subject. Do the correlation analysis. Results:As to the parameters of P300,the difference between myelinopathy and non-myelinopathy was prolonged latent phase(P<0.05) . The latency of the part of non-myelinopathy group prolonged slightly. Compared with the healthy group,there was significant deviation(P <0.05) . The EEG of the myelinopathy group was mainly expressed in δ-wave and θ-wave. The EEG of the nonmyelinopathy group was mainly expressed in α-wave and β-wave. The EEG of the healthy group was mainly expressed in α-wave. As to the apoprotein B,there was no difference in each group. Conclusion:The patients with mild cognitive impairment who have prolonged latent phase may convert to pristine vascular dementia. The increase of θ-wave in the pars temporalis may indicate pristine vascular dementia. Apoprotein B has no clinical significance for the diagnosis of pristine vascular dementia.更多还原
