抗苗勒管激素与多囊卵巢综合征相关性分析

【摘要】 目的研究多囊卵巢综合征(PCOS)血清抗苗勒管激素(AMH)水平及其与PCOS各项指标的相关性。方法选择2006年6~12月在中山大学附属第二医院妇产科就诊的47例PCOS患者为研究组,收集同期年龄匹配的40例健康女性为对照组,应用酶联免疫吸附(ELISA)法检测血清AMH。结果研究组的血清AMH明显高于对照组[(9.85±4.93)μg/L对(7.13±3.02)μg/L,P=0.002)。成年期研究组血清AMH亦明显高于对照组[(10.64±5.47)μg/L对(7.01±2.43)μg/L,P=0.005),而青春期研究组血清AMH虽较对照组增高,但差异无统计学意义[(8.95±4.18)μg/L对(7.26±3.58)μg/L,P=0.168)。Pearson相关分析显示血清AMH与年龄无相关性,与雄激素和其他内分泌等各项指标无相关性,与平均卵巢体积呈正相关(r=0.319,P=0.029),但与平均卵泡数目无相关性。结论PCOS患者的血清AMH明显增高,其与血清雄激素相关性不明显,提示AMH有可能通过某种机制在卵泡局部发挥作用,影响卵泡发育而参与PCOS的病理生理过程。

【Abstract】 Objective To detect the serum anti-mullerian hormone(AMH)level of patients with polycystic ovary syndrome(PCOS),and to explore the relationship between AMH and PCOS.Methods Totally 47 patients with PCOS and 40 healthy females were enrolled from June 2006 to December 2006 in the Second Affiliated Hospital of Sun Yat-sen University.Serum AMH levels were measured by enzyme linked immunosorbent assay(ELISA).Results The mean serum AMH level in PCOS group was significantly higher than that of the control group [(9.85±4.93)μg/L vs(7.13±3.02)μg/L,P=0.002],which was similar in adult PCOS patients and controls[(10.64±5.47)μg/L vs(7.01±2.43)μg/L,P=0.005].No difference was found in serum AMH of adolescent PCOS patients and controls [(8.95±4.18)μg/L vs(7.26±3.58)μg/L,P=0.168].Pearson correlation analysis showed positive relationship between serum AMH level and mean ovarian volume(r= 0.319,P=0.029).No significant relationships were found between serum AMH and age,serum androgen indexes,other hormone levels and mean follicle numbers.Conclusion Serum AMH level was elevated in patients with PCOS.No relationship is found between serum AMH and serum androgen indexes,suggesting AMH might affect follicular development as an autocrine factor and lead to the pathogenesis of PCOS.
