小儿体外循环术后毛细血管渗漏综合征的危险因素分析
Analysis of Risk Factors for Capillary Leak Syndrome in Children after Cardiopulmonary Bypass
【摘要】 目的探讨小儿先天性心脏病体外循环术后发生毛细血管渗漏综合征(CLS)的相关危险因素。方法回顾性分析2008年1~6月的375例先天性心脏病体外循环术后患儿的临床资料,对可能引起CLS发生的因素进行单因素分析和多元Logistic回归分析,确定与CLS发病相关的独立影响因素。结果本组资料体外循环术后CLS的发生率为6.13%,单因素分析显示CLS组的年龄、体重、心脏畸形种类、体外循环时间、术中最低温度明显比非CLS组低(P<0.01),经历二次转机、主动脉阻断时间延长以及有紫绀表现的患儿更倾向于术后出现CLS(P<0.05),经多元Logistic回归分析显示年龄因素是CLS发生的主要独立影响因素,体外循环时间、心脏畸形的种类、CPB术中最低温度是CLS发生的次要影响因素。结论年龄是CLS发生的主要独立影响因素,体外循环时间、心脏畸形的种类、CPB术中最低温度在CLS发生中具有一定意义。
【Abstract】 Objective To determine the risk factors of capillary leak syndrome(CLS) in children after cardiopulmonary bypass. Methods Clinical data were collected from children who underwent cardiopulmonary bypass in our hospital between January 2008 and June 2008. Several statistically significant influencing factors were screened out with univariate analysis and independent influencing factors of CLS were determined with multivarite logistic regression analysis. Results The morbidity rate of CLS was 6.13 %. Univariate analysis showed that age,weight,type of congenital heart disease,duration of cardiopulmonary bypass,minimal temperature of CPB were lower in patients with CLS than in those without CLS(P<0.01) . Second time CPB,duration of aortic clamping,cyanotic congenital heart defects were higher in patients with CLS than in those without CLS(P<0.05) . Logistic analysis showed the influencing factors of CLS were age,duration of cardiopulmonary bypass,type of congenital heart disease,and low temperature during CBP,age was the most important factor. Conclusion Age is the main independent influencing factor of the incidence of CLS,duration of cardiopulmonary bypass,type of congenital heart disease,and low temperature during CBP have certain significance in the incidence of CLS.
