尿沉渣微卫星DNA分析在膀胱癌早期复发监测中的应用价值
CLINICAL APPLICATION OF MICROSATELLITE DNA ANALYSIS OF URINARY SEDIMENT IN THE EARLY RECURRENT BLADDER TRANSITIONAL CELL CARCINOMA (BTCC)
【摘要】 [目的]研究尿沉渣微卫星DNA分析在膀胱癌早期复发监测中的应用价值。[方法]对51份膀胱癌术后随访尿液标本进行微卫星DNA序列分析,包括选择适合中国人种的13对微卫星位点的诊断组合,对尿沉渣和白细胞的DNA行PCR扩增、聚丙烯酰胺凝胶电泳及溴化乙锭(EB)染色,分析条带图像结果。随访同时进行膀胱镜复查(术后每3个月1次)。[结果]随访过程中复发17例,复发率为30%(17/51),膀胱癌复发患者中16例患者尿沉渣中有一个或多个微卫星位点有异常改变(包括LON和MSI),未复发患者中3例患者微卫星异常,两者差异有统计学意义。复发监测中尿沉渣微卫星异常改变早于膀胱镜下检查阳性表现(16.42±4.237)周。[结论]①尿沉渣微卫星DNA序列分析是监测膀胱癌术后早期复发的一个较好的无创性检查方法,有一定的临床应用价值。②在复发监测中微卫星序列改变早于膀胱镜下阳性表现。
【Abstract】 [Objective] To investigate clinical application of mircosatellite analysis of urinary sediment in the early recurrent BTCC. [Methods] 51 cases of BTCC were selected from postoperative patients, and a panel consisting of 13 mircosatellite loci which were confirmed to be suited to Chinese race was used. DNA from urine sediment and peripheral blood leukoes was analyzed by polymerase chain reaction (PCR) methods, then polyacrylamide gel electrophoresis (PAGE) and ethisium bromide (EB) stain PAGE. In course of follow-up, each case was examined by cytoscopy (One per 3 months). [Re-sults] 17 patients with BTCC were affirmed by cytoscopy. The recurrent rate was 30% (17/51). Among 16 recurrent patients with BTCC, they were detected to have mircosatellite alteration (LOH and MSI) in least one MS locus of urine sediment. In non-recurrent group, 3 patients had mircosatellite alteration, and it showed significantly different between them. The LOH and / or MSI in urine sediment revealed earlier among recurrent patients than positive cytoscopy, about ahead of 16.42±4.237 weeks. [Conclusion] 1. MS analysis of urine sediment is a favorable trauma-free method for detecting early recurrence of BTCC. 2. LOH and / or MSI in urine sediment is earlier than positive expression of cytoscopy.
