四川省盐亭县食道癌发病影响因素分析
A CASE-CONTROL STUDY ON THE RISK FACTORS OF ESOPHAGEAL CANCER IN YANT-ING COUNTY
【摘要】 [目的]探讨四川省盐亭县的食道癌发病影响因素,为该地区的食道癌预防工作提供依据。[方法]在盐亭县40~69岁的4038名调查对象中,将食道碘染色内镜检查正常者作为对照,病理活检结果为重增/原位癌、黏膜内癌及浸润癌作为病例,进行面对面问卷调查,并运用非条件Logistic回归,分析食道癌发病影响因素。[结果]食道癌发病危险因素有:吸烟(OR=2.250,OR95%CI:1.386~3.651)、配偶患消化系统其他肿瘤(OR=13.414,OR95%CI:1.168~154.011)、一级亲属患食道癌(OR=2.583,OR95%CI:1.240~5.382)、一级亲属患胃癌(OR=3.533,OR95%CI:1.399~8.918)。[结论]四川盐亭的食道癌高发不仅与饮食因素、生活习惯有关,还与食道癌、胃癌等消化系统肿瘤家族史有关。
【Abstract】 [Objective] To explore the risk factors of esophageal cancer in Yanting county, and to provide basic data for early prevention. [Methods] 4 038 residents aged from 40 to 69 were examined by endoscopy of esophageal and surveyed by face-to-face questionnaire. People with normal esophageal mucosa served as control group. By means of pathogenic examination, patients with esophageal mucosa sever hyperplasia and above served as disease group. Non-conditional logistic regression model was used to analyze the risk factors. [Results] Smoking increased the risk of esophageal cancer (OR = 2.250, OR95%CI: 1.386-3.651), first-degree relatives suffering from esophageal cancer (OR = 2.583, OR95%CI: 1.240-5.382) and stomach cancer (OR = 3.533, OR95%CI: 1.399-8.918), spouse suffering from other digestive system cancer (except stomach and esophagus cancer) were also risk factors (OR = 13.414, OR95%CI: 1.168-154.011). [Conclusion] The high incidence of esophageal cancer in Yanting county is relative not only to dietary and habits factor, but also to the family history of esophageal cancer and stomach cancer.
