【摘要】 目的研究血清瘦素、肿瘤坏死因子(TNF-α)与慢性阻塞性肺疾病(COPD)患者营养状况的关系。方法观察COPD临床稳定期患者148例,对照组30例。根据营养指标分为COPD营养不良组(COPDⅠ组)47例,COPD非营养不良组(COPDⅡ组)101例。采用双抗体夹心ELISA法检测血清瘦素、TNF-α。结果COPDⅠ组体重指数(BMI)、理想体重百分比(NW%)、三头肌皮皱厚度(TSF)、上臂中点臂围(MAC)、血清白蛋白(ALB)、总淋巴细胞(LYM)、瘦素水平与COPDⅡ组、对照组比较差异均有统计学意义(P<0.01),COPDⅠ组、Ⅱ组TNF-α水平分别与对照组比较差异均有统计学意义(P<0.01)。BMI、NW%、TSF、TNF-α与瘦素水平呈正相关(P<0.01)。结论COPD患者血清瘦素水平与BMI、NW%、TSF及TNF-α相关,血清瘦素水平受到TNF-α系统调节,其协同作用可能与COPD患者营养不良的发生有关。更多还原

【Abstract】 Objective To investigate the correlation between malnutrition,tumor necrosis factor-α(TNF-α)and the serum leptin level in the patients with chronic obstructive pulmonary disease(COPD).Methods 148 COPD patients in stable stage and 30 normal controls were studied.COPD patients were divided into malnutrition group(groupⅠ) and non-malnutrition group(group Ⅱ) according to the nutritional parameters.Serum leptin level and TNF-αwere measured by ELISA.Results There were significant differences on BMI,NW%,TSF,MAC,ALB,LYM between groupⅡ,control group and groupⅠ.There were significant difference on serum leptin between groupⅡ,control group and groupⅠ.There were significant difference on TNF-αlevels between groupⅠ,groupⅡ and control group.Serum leptin in groupⅠ and groupⅡ showed a positive correlation with BMI,NW%,TSF and TNF-α.Conclusion Serum leptin correlate with BMI,NW%,TSF and TNF-αin COPD patients.Serum leptin levels were adjusted by TNF-α system.The synergy may be related to the occurrence of malnutrition in patients with COPD.更多还原
