【摘要】 目的自知力缺乏是精神分裂症服药依从性低下的主要原因,但影响自知力因素众多,本文则就相关因素进行了研究。方法以满足CCMD-3精神分裂症诊断标准,年龄18~60周岁之间,无精神发育迟滞和各种药物依赖的93例入院患者为对象,采用自知力评定量表(SAI)评价入院时和治疗6周后的自知力认识水平。分析SAI与BPRS的相关性,首发和复发患者及相关职业/文化背景、抗精神病药物不同代类等对SAI的影响。结果入院时BPRS分与SAI分呈现背离状态,显示出明显负相关(rs=0.08,P>0.05)。但随着6周治疗后BPRS分下降,SAI分上升,两者则呈现出了显著相关(rs=0.3,P<0.05)。在受教育程度,职业背景及抗精神病药物种类与自知力的单因素不同水平差异分析中,均未显示差异有统计学意义,而且在第1、2代或2代抗精神病药物联用治疗后的BPRS分和SAI分也显示出明显负相关(P均>0.1)。结论精神分裂症的自知力提高主要得益于精神症状的有效缓解,而其它因素和药物种类的影响甚微。更多还原

【Abstract】 Objective The main reason for the lower medication compliance in schizophrenic is lack of insight,and there are many influencing factors,we will explore these relative factors.Methods 93 patients(aged from 18 to 60 years)without mental retardation and drug-dependent were diagnosed as schizophrenia in accordance with the CCMD-3 diagnosis standard.The schedule for assessment of insight(SAI)was used to assess the awareness of insight from baseline to week 6,then we studied the correlation betweeo SAI and BPRS,investigate the impact of the first and recurrent cases,related professional,cultural backgrounds and drug category on SAl.Results The BPRS scores were deviated from SAI scores at the beginning,showed significant negative correlation(rs=0.08,P>0.05).But after 6 weeks treatments,BPRS scores dropped and SAI scores rised,showed significant correlation(rs=0.3,P<0.05).Among the analysis of level of education,professional background,antipsychotics category and single-factor of insight,the results did not show significant difference,but also showed significant negative correlation(P>0.1)after treated by the first or second generation antipsychotics and combined treatment.Conclusion The improvement of psychiatric symptoms increases the insight of schizophrenics,the other factors and drug category have little effect on.更多还原
