【摘要】 目的探讨诱导化疗联合同期放化疗治疗局部晚期鼻咽癌的安全性有效性。方法收集2003年10月～2007年8月56例局部晚期鼻咽癌患者(Ⅲ,Ⅳa,92分期),于同期放化疗前接受2～3个疗程的诱导化疗,观察治疗效果。结果56例患者所有的患者完成了计划剂量的治疗。诱导化疗结束后,鼻咽完全缓解率为19.6%,颈部淋巴结完全缓解率为31.6%。放疗结束3个月后,鼻咽完全缓解率为98.2%,颈部淋巴结完全缓解率为94.7%。以PF方案行诱导化疗患者耐受性好,同期放化疗期间,37.5%患者发生了3度粘膜炎,18%患者发生了3度呕吐反应。中位随访30个月后,2年总的生存率(OS)为92.8%。结论以诱导化疗联合同期放化疗的方式治疗局部晚期鼻咽癌方式,患者耐受性好,且能获得较好的治疗效果。更多还原

【Abstract】 Objective To investigate the safety and efficacy of neoadjuvant chemotherapy followed by concurrent chemoradiotherapy (CCRT) on patients with locoregional nasopharyngeal carcinoma at advanced stage. Methods From Oct. 2003 to Aug. 2007, 56 patients with locoregionally NPC (Ⅲ, Ⅳa)were enrolled. Before CCRT, patients were received two to three cycles of chemotherapy (PF) with cisplatin (80～100mg/m2), followed by 48 hours infusion of 5-Fu (3.0g/m2) every three weeks. Then CCRT with cisplatin (80 mg/m2 )was given on day 1, 22,and 43 from the first day of standard radiotherapy. Results All patients completed planned treatment and were eligible for toxicity and response analysis. After neoadjuvant chemotherapy, 19.6% patients achieved complete response (CR) in nasopharynx and 31.6% patients achieved CR in regional nodes; Three months after RT, 98.2% patients achieved CR in nasopharynx and 94.7% patients achieved CR in regional nodes. Neoadjuvant PF was well tolerated and the most common acute high-grade toxicity of CCRT was grade 3 mucositis (37.5%) and grade 3 vomiting (18%). At median follow-up of 30 months, the 2-year overall survival rates were 92.8%. Conclusion This strategy is well tolerable and the results were satisfactory.更多还原
