口服葡萄糖耐量试验静脉留置针采血溶血的影响因素分析及采血方案的探讨

【摘要】 目的观察口服葡萄糖耐量试验(OGTT)经静脉留置针采血,不同管径的静脉留置针、不同采血部位、不同的注血方法以及采血的难易程度对溶血的影响,从而探索出OGTT静脉留置针采血的最佳组合方案。方法将75例志愿者分别采用不同管径的留置针、不同采血部位、不同的注血方法,于空腹及口服葡萄糖后0.5、1、2、3h时采血,经离心后比较各组标本的溶血发生率,并进行单因素分析和多因素Logistic回归分析。结果经肘部采血、采用20G管径静脉留置针采血、采血容易者,溶血发生率均明显降低,与前臂采血、采用22G管径静脉留置针采血、采血困难者比较差异有统计学意义(P<0.01)。两种注血方法的溶血发生率差异无统计学意义(P>0.05)。多因素Logistic回归分析显示,采血部位、静脉留置针管径与溶血发生率有关(P<0.01)。结论经肘部采血和采用20G管径静脉留置针是避免溶血的最佳组合方案。

【Abstract】 Objective To study factors affecting hemolysis in blood samples drawn from intravenous catheter in the oral glucose tolerance test(OGTT),so as to explore an optimum combination scheme for OGTT blood sampling.Methods Seventy-five volunteers underwent blood sample drawn from intravenous catheter on fasting and 0.5,1,2,3h after oral glucose respectively through different gauze catheters,blood sampling cites,and blood injection methods.The hemolytic rates of centrifuged blood samples in each group were compared,and univariate analysis and multiple Logistic regression were conducted.Results The incidence rate of hemolysis was remarkably lower when blood sample was easily done by applying 20-gauge intravenous catheter via antecubital site,there was a significant difference as compared with difficult blood sampling by applying 22-gauze catheter via forearm site(P<0.01).There was no significant difference in hemolysis rate between the two ways of blood injection(P>0.05).multiple Logistic regression analysis showed that hemolysis accidence rate was associated with blood sampling site and the gauze of intravenous catheter(P<0.01)Conclusion Antecubital blood sampling with 20-gauge intravenous catheter is the optimum combination scheme to avoiding hemolysis.
