彩色多普勒血流显像对原发性甲状腺功能亢进症与桥本甲状腺炎的鉴别诊断
Differential diagnosis of primary hyperthyroidism and hashimoto thyroiditis by color Doppler flow imaging
【摘要】 目的探讨彩色多普勒血流显像对原发性甲状腺功能亢进症(甲亢)和桥本甲状腺炎患者甲状腺的诊断价值。方法超声观测38例甲亢、21例桥本甲状腺炎患者及35例健康者为对照组的甲状腺的形态、内部回声、大小、血流分布、甲状腺上动脉收缩期血流峰值速度和阻力指数,同时测定患者心率。结果甲亢组和桥本甲状腺炎组的甲状腺体积均较对照组增大(P<0.01),但二者差异无统计学意义。甲亢组甲状腺回声呈密集均匀增强型和粗大不均匀增强型;桥本甲状腺炎组甲状腺回声降低,内部有网格状条索状强回声。甲亢组与桥本甲状腺炎组甲状腺实质内部血流均弥漫性增多,甲状腺上动脉血流速度均较对照组加快(P<0.01),但前者加快明显(P<0.01),三组阻力指数差异无统计学意义。结论彩色多普勒血流显像有助于甲亢与桥本甲状腺炎的诊断和鉴别诊断。
【Abstract】 Objective To explore the diagnostic value of color Doppler flow imaging in patients with primary hyperthyroidism and Hashimoto thyroiditis. Methods Thirty-eight patients with primary hyperthyroidism,21 patients with Hashimoto thyroiditis and 35 healthy subjects were enrolled in this study. The morphous of thyroid,internal echo,size,blood distribution,the blood flow parameters of the superior thyroid artery including the maximum blood flow velocity(Vmax),heart rate (HR) and resistance index (RI) were measured by color Doppler flow imaging. Results Compared with control group,the size of thyroid in primary hyperthyroidism group and Hashimoto thyroiditis group was increased (P<0.01),but there was no significant differences between the two groups. The echo of the thyroid enhanced in primary hyperthyroidism group and decreased in Hashimoto thyroiditis group with grid strong echo interior. Thyroid glandular substance internal blood of two groups was increased diffusely. Compared with control group,the blood flow of superior thyroid artery was sped up (P<0.01). The Vmax and HR were higher in the patients with primary hyperthyroidism than that with Hashimoto thyroiditis (P<0.01). There was no significant differences on RI(P>0.05) in three groups. Conclusion CDFI is a valuable method for differential diagnosis of primary hyperthyroidism and Hashimoto thyroiditis.
