【摘要】 目的:通过比较分析寻求一种相对较好的靶肺灌注试剂,有利于功能性肺切除的进一步研究。方法:32只健康家兔随机分成4组,每组8只,以灌注试剂生理盐水(A组)、碘油(B组)、平阳霉素(C组)、平阳霉素-碘油乳剂(D组)命名分组;分别灌注相应试剂于右下肺叶后留下侧位胸片,再用骨水泥封堵靶肺支气管开口,在第30天切取靶肺标本,观察大体形态,利用标本行HE染色进行病理分析,再用Western-blot法检测MMP-2、MMP-9表达蛋白量,分析组间差异。结果:B、C、D组靶肺呈苍白色或灰白色,表面见出血斑点,组织有不同程度硬化,体积缩小,B组病理切片上可见少量纤维增生,有散在的坏死灶及急性渗出液,MMP-2、MMP-9表达蛋白量升高,C、D组靶肺肺泡间隔增宽,纤维组织大量增生,肺泡结构破坏明显,部分区域可见较大的含气气泡,两组的MMP-2、MMP-9表达蛋白量相对其他组明显升高(P<0.05),对照组A组靶肺肿胀,体积增大,肺泡腔内积聚淡红色渗出液,其中含有较多的各种炎性细胞。结论:平阳霉素组、平阳霉素-碘油乳剂组靶肺比碘油组靶肺具有相对稳定的组织结构,肺减容效果较好;而平阳霉素-碘油乳剂为较合适的灌注试剂。更多还原

【Abstract】 Objective: To search the best filling agent of target lung for the research of functional pulmonary lobectomy(FPLT).Methods: Thirty-two healthy rabbits were divided into four groups randomly as control group(group A,n=8),iodine oil group(group B,n=8),pingyangmycin group(group C,n=8),and emulsion of iodine oil and pingyangmycin group(group D,n=8),according to the right lower lobe filled with different agents respectively,then the target bronchus was occluded with polymethylmethacrylate(PMMA).Animals in each group were killed on the 30th day and target lungs were harvested.Samples were routinely stained by hematoxylin and eosin(H&E) for assessing the general histological appearance of lung tissue,and the MMP-2 and MMP-9 expressions were assayed by Western-blot.Results: In group B,C,and D,the color of target lungs was pale or gray,the volum became smaller,and there were many bleeding points on the surface.In group A,target lungs were swellen and their volum became bigger.Histologically,there were many small abscesses in group B and less collagen fibers deposited in the interstitial tissue when compared to group C and D.The expressions of MMP-2 and MMP-9 were significantly higher in group C and D than in group A and B respectively(all P<0.05),but there were no difference between group C and D.Conclusion: Group C and D have more stable tissue structure than group B,and emulsion of iodine oil and pingyangmycin is the best filling agent for functional pulmonary lobectomy.更多还原
