急性脑血管病患者血钠、血钾的变化及其临床意义
Changes of serum sodium,serum potassiun in patients with acute cerebrovascular diseese and its clinical significance
【摘要】 目的探讨急性脑血管病患者血钠、血钾的早期改变与其中风类型、病情轻重的关系以及对病人预后的影响。方法全部病人558例,其中男339例,女219例;出血性脑血管病225例,其中脑出血215例,蛛网膜下腔出血10例;缺血性脑血管疾病333例,其中脑血栓形成243例,出血性脑栓塞7例,腔隙性脑梗塞83例。病人入院后次日晨、3d后及一周后分别空腹抽取静脉血3ml,进行血钠、血钾测定。结果采用x2检验、Ridit分析及R×C表计数资料假设检验。结果血钠的高低与中风类型、病情的轻重及预后有关,低血钾与中风类型无关外,血钾的异常与病情的轻重及预后有关。结论因为急性脑血管病的病情轻重及预后与电解质紊乱有关,因此在今后临床工作中我们可以把电解质的指标作为判断急性脑血管病患者的病情及预后的指标之一。同时它提示我们在今后的临床工作中应该严格观察病人电解质变化情况,并积极进行纠正,以减少疾病的死亡率,促进康复。
【Abstract】 Objective To investigate the relation between sodium and kalium early change in the blood and apoplexy type and illness degree of the acute cerebrovascular disease sufferer,and the prognostic effect on the patients. Methods The total number of patients were 558,included 339 male and 219 female. Among these patients,215 were with cerebral hemorrhage,10 were with subarachnoid hemorrhage,and the other 333 were with ischemic cerebrovascular disease which including 243 cerebral thrombosis,5 hemorrhagic cerebral embolism and 80 lacunar infarction. The whole patients were phlebotomized 3ml with limosis to check up the concentration of the sodium and potassium of the blood three times respectively in the next morning,3 days and 1 week after they entered in hospital. Results High or low sodium concentration was not only related to the types and seriousness of strokes,but prognosis as well. While hypokalemia had no reference to the variety of strokes,and the abnormal of the potassium concentration in the blood were in connection with seriousness and prognosis of strokes. Conclusion Due to the concentration of the blood sodium has relation with the type of apoplexy,serious degree of the pathogenetic condition and the prognosis,meanwhile,the concentration of the blood potassium are concerned with serious degree of the pathogenetic condition and prognosis but has no independence with the type of apoplexy. We can conclude that severity of the illness and prognosis are concerned with electrolyte disturbance,so the change of the electrolyte can be regarded as an index to determine the prognosis of the acute cerebrovascular disease.
