无创血流动力学指标在慢性心力衰竭诊治中的临床应用
Clinical application analysis of noninvasive hemodynamics index in treating chronic heart failure
【摘要】 目的分析无创血流动力学指标在慢性心力衰竭(CHF)诊治中的临床应用价值。方法选择住院的CHF患者80例,采用数字化无创血流动力学监测系统测量心率(HR)、动脉血压(ABP)、每搏输出量(SV)、加速度指数(ACI)、速度指数(VI)、胸腔液体量(TFC)等指标;将无创血流动力学指标与心功能分级及心脏彩超LVEF结果进行对比,同时对治疗前后的结果进行比较。结果TFC指标心功能II、III、IV级之间两两比较均存在统计学差异(P<0.05),ACI指标则在三组心功能分级之间均不存在统计学差异(P>0.05);治疗前后对比结果表明,SV、每搏指数(SI)、心输出量(CO)、心脏指数(CI)等诊断心衰的指标治疗后均显著上升,判断后负荷的阻力指数(SVRI)、外周血管阻力(SVR)治疗后也显著下降,TFC在治疗前后也存在明显的统计学差异(P<0.05),VI、ACI指标在治疗前后无明显改变(P>0.05)。结论无创动力学指标中的TFC是心力衰竭程度诊治最敏感的指标;部分CHF患者需要经历较长时间的治疗才能获得收缩功能的改善,进而在无创血流动力学指标中得到体现。
【Abstract】 Objective To analyze the clinical application value of noninvasive hemodynamics index in treating chronic heart failure.Methods 80 CHF inpatients were chosen for measuring heart rate(HR),arterial blood pressure(ABP),stroke volume(SV),acceleration index(ACI),velocity rate(VI)and thoracic fluid capacity(TFC) by digital noninvasive hemodynamics monitoring system.The results of noninvasive hemodynamics index,cardiac function classification and cardiac color ultrasound LVEF were compared.The results before and after treatment were compared too.Results There were statistical difference(P<0.05) in II,III and IV class of TFC indicators of cardiac function.There were no statistical difference(P>0.05) of ACI index in three classes.The comparison result before and after treatment showed that indexes of stroke volume(SV),stroke volume index(SI),cardiac output(CO) and cardiac index(CI) to assist the diagnosis of heart failure increased significantly after treatment.Resistent index(SVRI) and systemic vascular resistance(SVR) decreased significantly after assisting judgment.There were statistical difference(P<0.05) in TFC.There were no significant change of VI and ACI before and after treatment(P>0.05).Conclusion TFC in noninvasive hemodynamics index was the most sensitive index in the diagnosis of the extent of heart failure.A part of CHF patients needed a long treatment improvement to get systolic function.And the effect could be reflected in noninvasive hemodynamics index.
