【摘要】 目的探讨血清肿瘤标记物对胰腺癌诊断及随访的临床价值。方法对53例胰腺癌患者和39例胰腺良性疾病患者进行四项血清肿瘤标记物检测,并对18例胰腺癌术后患者进行随访。结果胰腺癌组血清肿瘤标记物水平均高于胰腺良性病组(P<0.05),4项血清肿瘤标记物中以CA199敏感性和特异性最高,分别为71.69%和74.35%,联合检测可提高其敏感性和特异性,分别为88.69%和89.74%。胰腺癌未切除的患者肿瘤标记物水平术后未降至正常,胰腺癌切除后肿瘤标记物水平降至正常的患者在随访中再升高后出现肿瘤复发或转移,且肿瘤标记物在影像学检查证实复发数月前已出现升高。结论血清肿瘤标记物联合检测可提高胰腺癌的诊断率,有助于早期胰腺癌诊断,并可用于判断治疗效果和预后、监测肿瘤复发和转移。更多还原

【Abstract】 Objective To evaluate the clinical value of markers in serum detected for diagnosis and the follow up in pancreatic cancer.Methods The 4 tumor markers levels were detected in 53 patients with pancreatic cancer and 39 patients with benign pancreatic diseases,and out of 53 patiets with pancreatic cancer,18 cases were followed up after operation.Results The mean levels of tumor markers in pancreatic cancer group were higher than group of benign pancreatic diseases(P<0.05).Sensitivity and specificity of CA199 out of 4 tumor markers was the highest respectively 71.69%and 74.35%.Sensitivity and specificity could be improved by detection combined with 4 tumor markers in serum respectively 88.69%and 89.74%.while the mean levels of tumor markers did not decrease to normal after operation in patients without resection of cancer,in other patients with pancreatic cancer,the mean levels of tumor markers decreased to normal after resection of cancer,and recurrence and metastasis appeared after high levels of tumor markers again during the followed up.Several months before recurrence and metastasis confirmed by imaging.Conclusion The detection combined with 4 tumor markers in serum could be used for improving diagnosis rate,early diagnosis,evaluating therapeutic effect,decision of prognosis and monitoring recurrence and metastasis of pancreatic cancer.更多还原
