米非司酮合并米索前列醇对早孕妇女性激素水平和体液免疫功能的影响

【摘要】 目的:探讨米非司酮合并米索前列醇对早孕妇女性激素水平和体液免疫功能的影响。方法:用化学发光法检测服药前后外周血中雌二醇(E2)、孕酮(P)、睾酮(T)、催乳素(PRL)、促黄体生成素(LH)、卵泡刺激素(FSH)和绒毛膜促性腺激素(β-HCG)水平。用免疫化学自动分析仪检测服药前后外周血IgA、IgG、IgM、C3和C4含量。结果:服药期间P、β-HCG无明显变化,E2有所升高,孕囊排出后均显著下降。β-HCG变化与P、E2变化相关(r=0.98、0.95)。FSH在服药期间略有下降,第6、8天又有所回升。T在第6天、第8天有所下降。PRL和LH服药前后波动不大。服药后第72 h IgA、IgM水平有所下降,与服药前相比下降有显著的统计学意义(t=3.28-4.89,P<0.05),第8天IgM仍在低水平。结论:米非司酮合并米索前列醇终止早孕过程中,β-HCG、P和E2继发性降低。米非司酮可能影响早孕妇女IgA、IgM水平,对IgG、C3和C4无明显影响。

【Abstract】 Objective:To investigate the effect of mifepristone on the levels of gonadal hormones and humoral immunity in early pregnancy.Methods:The levels of E2,P,T,PRL,FSH,LH and β-HCG of peripheral blood in different days in mifepristone-treated early pregnancy were measured by chemiluminescence means.The expressions of IgA,IgG,IgM,C3 and C4 were assessed by immunochemistry autoanalyser.Results:The levels of P and β-HCG had no obviously changes during administration,E2 had increased a little,then the levels of P,E2 and β-HCG sharply decreased at H72.Pearson correlation coefficients of β-HCG with P was 0.98,that of β-HCG with E2 was 0.95.The levels of FSH decreased slowly during administration,then slightly rose at D6 and D8.The level of T decreased a little at D6 and D8.There was no appreciable change of either PRL or LH.The levels of IgA and IgM were lower in H72 than H0,there was significant difference(t=3.28~4.89,P<0.05).The level of IgM was still low in D8.Conclusion:In the course of the termination of early pregnancy with mifepristone and misoprostol,β-HCG decrease,then P and E2 decrease. Mifepristone might affect the levels of IgA and IgM,but has no obviouse effect on IgG,C3 or C4.
