【摘要】 目的:总结人工关节置换术治疗高龄患者股骨粗隆间不稳定型骨折的方法及临床疗效。方法:对2003年2月～2006年10月采用人工关节置换术治疗的47例高龄股骨粗隆间不稳定型骨折患者进行随访分析。其中男21例,女26例;年龄75～94岁,平均年龄82.5岁;骨折按Evan’s分型IIIA型13例,IIIB型23例,IV型11例。7例采用全髋关节置换术治疗,40例合并有其他器官疾病不能耐受长时间手术患者采用股骨头置换术治疗。结果:1例术后5个月死于脑血管意外,1例术后16个月因心脏疾病死亡,46例获6～35个月随访,39例(84.8%)患者髋关节功能接近或恢复到受伤前水平,未出现感染、松动、脱位等并发症。结论:人工关节置换术治疗高龄患者股骨粗隆间不稳定型骨折能迅速恢复患肢功能,减少并发症的发生,显著提高患者的生活质量,是治疗该类骨折的有效方法之一。更多还原

【Abstract】 Objective:To summarize the method and clinical effect of hip joint replacement for the intertrochanteric fractures in senile patients.Method:From February 2003 to October 2006,47 unstable intertrochanteric fractures patients,including 21 male and 26 female,with an average age of 82.5 years(range,75～94),were treated with hip joint rephcement.According to the Evan’s classification,there were typeⅢA in 13 cases,ⅢB in 23 cases,Ⅳin 11cases.The clinical results were analyzed.7 were treated by total hip joint replacement while 40 patients suffering from other organ illness were treated by femoral head replacement because of they,were incapable of enduring long time operation.Results:One case died through cardiac accident and one case died through cerebral vascular accident.46 cases were followed up for 6 to 35 months.The joint function returned to normal or near normal was found in 39 cases(84.8%).No infection,loosening and dislocation was found.Conclusions:Hip joint rephcement may be an effective method for the treatment of senile patients with unstale intertrochanteric fractures with earlier functional recovery and low incidence rate of complications,which improve the quailty,of life in senile patients.更多还原
