【摘要】 目的以肾小球滤过率(GFR)为标准,评价 Cockcroft-Gault、肾脏病膳食改良研究(MDRD)、简化 MDRD 及 Martin 公式在评估肾功能损伤中的价值。方法选择120例行 GFR 测定的非透析患者,以 MDRD、简化 MDRD 及 Martin 公式推算 GFR,以 Cockcroft-Gault 公式推算肌酐清除率,比较各推算值与 GFR 的差异及相关性。结果 Cockcroft-Gault 公式推算值与 GFR 值差异有显著性。MDRD、简化 MDRD 和 Martin 公式推算值仅在 GFR<80 ml·min-1·(1.73 m2-1时与 GFR 差异无显著性,但只有在 GFR<50 ml·min-1·(1.73 m2-1时与 GFR 有相关性。结论 Cockcroft-Gault 公式不适用于评价中国人肾功能受损的情况。MDRD、简化 MDRD 及 Martin 公式仅在 GFR<50 ml·min-1·(1.73 m2-1时可用于粗略评价中国人肾小球滤过功能受损情况。

【Abstract】 Objective The study was designed to compare the Cockcroft-Gauh formula,modifica- tion of diet in renal disease study(MDRD)formula,simplified MDRD formula and Martin formula in the estimation of glomerular filtration rate(GFR)to assess their validity in Chinese subjects.Methods One hundred and twenty patients without hemodialysis were included.The GFR estimated using Coekcroft-Gauh formula,MDRD formula,simplified MDRD formula and Martin formula were compared with GFR assessed by Gate’s method.Results GFRs estimated by Cockcroft-Gauh formula were significantly different from that by the standard technique.GFRs estimated by MDRD formula,simplified MDRD formula and Martin formula were not significantly different from that of Gate’ s method,provided that the GFR was lower than 80 ml·min-1·(1.73 m2-1,but significant relationships were only existed among GFRs estimated by MDRD,simplified MDRD,Martin and by Gate’s method in case the GFR was lower than 50 ml·min-1· (1.73 m2-1.Conclusions When the GFR is lower than 50 ml· min-1·(1.73 m2-1,the MDRD, simplified MDRD and Martin formula may be used for their estimation among Chinese.But Cockcroft-Gault formula may not be suitable for the estimation.
