【摘要】 目的研究乌司他丁对心肺复苏患者血液流变学及其预后的影响。方法随机选择2002年1月～2006年8月急诊科的126例自主循环恢复的心肺复苏患者作为研究对象,分成两组:A组在自主循环恢复后除采用常规的支持治疗外加用乌司他丁,B组采用常规的支持治疗。所有患者均在自主循环恢复后2h、24h、72h、第7d查血液流变学指标,同时观察患者的病死率、所需肾脏替代治疗的例数、所需机械通气治疗的例数、所需血管活性药物支持的例数,抗菌药使用时间超过两周的例数、存活出院患者的住院时间等指标。结果自主循环恢复后2h、24h两组的血流变指标无明显的差异(P>0.05),但72h、第7d的差异明显(P<0.05,P<0.01)。乌司他丁治疗组的病死率、存活患者的住院时间、所需肾脏替代治疗的例数、所需机械通气治疗的例数、所需血管活性药物支持的例数均较常规组明显的减少或降低(P<0.05)。结论乌司他丁改善心肺复苏患者的血液流变学指标,缩短存活患者的住院时间、保护重要脏器的功能,降低心肺复苏患者病死率。更多还原

【Abstract】 Objective To investigate the hemorheology effect and prognosis of ulinastatin in cardiopulmonary resuscitation patients.Methods A Prospective randomized controlled study was performed involving 126 restora-tion of spontaneous circulation(ROSC) patients admitted to emergency department during January 2002 to August 2006.On admission,patients were randomly assigned to two groups:group A using routine supportive treatment and ulinastatin,group B using routine supportive treatment.Hemorheology index of each patient was collected after restoration of spontaneous circulation(ROSC),at 2,24,72 hour and 7 day,meanwhile observating the mortality,the length of stay in the hospital for survival,the proportion of patients who needed renal replacement therapy,ventila-tory support,inotropic or vasopressor support,treatment with antibiotics exceeding two weeks.Result Hemorheology index at 2,24 hour after ROSC has no significantly difference in the two groups,but significant changes were found at 72 hour and 7 day(P<0.05 and P<0.01).Mortality is lower,the length of stay in the hospital for survival,the proportion of patients who needed renal replacement therapy,ventilatory support,inotropic or vasopressor support,treatment with antibiotics exceeding two weeks were less or lower in group A than in group B(P<0.05).Conclusion Ulinastatin improves hemorheology index,reduces motality and shortens the length of stay in the hospital for survival,protects vital organ function in cardiopulmonary resuscitation patients.更多还原
