【摘要】 目的探讨牙周洁治和刮治对牙龈血流量(GBF)、血流速度(BCV)和血浓度(BC)等微循环指标和牙周临床指标的影响。方法选慢性牙周炎(CP)患者20例、侵袭性牙周炎(AgP)和牙龈炎(G)患者各10例,选健康人(H)10例作为对照,用激光多普勒血流仪测定牙周洁刮治前后患者的GBF、BCV和BC及牙龈指数(GI)、菌斑指数(PLI)、牙周探诊深度(PD)、附着丧失(AL)的变化,并与健康组比较。结果治疗后1周时CP组、AgP组和G组患者GBF和BCV较治疗前均明显增加,BC较治疗前明显降低,CP组患者GBF,BCV和BC值与AgP组有非常显著性差异(P<0.01);治疗后CP组,AgP组和G组GI和PLI明显降低,CP组和G组PD和CP组的AL明显降低,AgP组PD和AL轻度降低;治疗后1个月时各组GBF,BCV和BC和牙周临床指标与治疗后1周时无显著性差异。结论牙周洁治和刮治对牙龈微循环指标有明显改善作用,其临床治疗作用与牙龈微循环改善密切相关。更多还原

【Abstract】 Objective To study the biological effects of periodontal scaling on gingival blood flow(GBF)、blood current velocity(BCV) 、blood concentration(BC) in gingiva and the effects on different clinical indices.Methods Twenty cases of chronic periodontitis(CP) 、10 cases of aggressive periodontitis(AgP) 、10 cases of gingivitis(G) and 10 cases of health (H) cases were selected, and the GBF,BCV and BC value in gingiva of each group was measured before and after periodontal scaling by laser doppler flowmeter type 3, and also the difference of gingival index(GI), plaque index(PLI), probing depth(PD) and attachment loss(AL) of different group was measured and analysed between periodontal scaling before and after 1 week and 1 month.Results The GBF and BCV value in gingiva of group CP,group AgP and group G were increased more significantly after periodontal scaling 1 week than before, and the BC value was reduced more than periodontal scaling before. Very significant difference of GBF、BCV and BC after periodontal scaling 1 week were seen between the group CP and group AgP ( P < 0.01 ). The GI and PLI of group CP, group AgP and group G after periodontal scaling 1 week were lowered markedly than that of before, the PD value of group CP and group G, and the AL value of group CP were reduced more severely, but the PD and AL of group AgP were reduced less. There were no significant differences of the GBF, BCV, BC value and clinical indices of group CP, group AgP and group G between periodontal scaling after 1 week and after 1 month.Conclusion Periodontal scaling plays a important role in improving the gingival microcirculation which is related to improving clinical curative effect.更多还原
