骨髓基质细胞移植治疗缺血性脑损伤
Bone marrow stromal cell transplantation in the treatment of ischemic brain damage
【摘要】 目的:从国外骨髓基质细胞移植治疗缺血性脑损伤的发展现状出发,探讨骨髓基质细胞移植治疗缺血性脑损伤的可行性和可能机制。资料来源:应用计算机检索PUBMED 1992-01/2004-01期间的相关文章,检索词为“Marrow stromal cell,cerebral ischemia”,并限定文章语言种类为English。资料选择:对资料进行初筛,并查看每篇文献后的引文。纳入标准:①文章所述内容应与骨髓基质细胞移植治疗缺血性脑损伤研究相关。②同一领域的文献则选择近期发表或权威期刊杂志的文章。排除标准:重复研究或Meta分析类文章。资料提炼:共收集到208篇相关文献,42篇文献符合纳入标准,排除的166篇文献为内容陈旧或重复。符合纳入标准的42篇文献中,17篇涉及骨髓基质细胞的定义和生物学特性,12篇涉及骨髓基质干细胞的神经分化潜能,9篇涉及骨髓基质干细胞神经分化的可能机制,14篇涉及骨髓基质干细胞的移植途径,15篇涉及骨髓基质干细胞对于缺血性脑损伤的治疗作用。资料综合:如何改善脑损伤后神经细胞的再生和神经行为的恢复,一直是困扰神经科医生的一大难题。科学家们已经在基质细胞移植治疗动物脑缺血方面做了很多工作,证明该方法是有效和可行的,成体动物的骨髓中存在一定数量的骨髓基质干细胞,其在适当条件下可以向神经干细胞分化。骨髓基质干细胞诱导分化为神经元样细胞的研究工作已有一定突破,如何延长骨髓基质干细胞源性的神经元样细胞的寿命是极为重要的。另一方面骨髓基质干细胞移植治疗缺血性脑损伤己取得一定的进展,如将骨髓基质干细胞在体外分化为神经元样细胞,再行细胞移植将会有事半功倍的效果。结论:骨髓基质干细胞移植治疗动物缺血性脑损伤研究己取得了一系列可喜的成果,其作用机制及临床应用有待深入探讨。
【Abstract】 OBJECTIVE: On the basis of the development of bone marrow stromal cell (BMSC) transplantation in treatment of ischemic brain damage, the feasibility and possible mechanism of the treatment is explored. DATA SOURCES: An online search of PubMed database was undertaken to identify the articles about using BMSC transplantation in treatment of ischemic brain damage published in English from January 1992 to January 2004 by using the deywords of "Marrow stromal cell, cerebral ischemia". STUDY SELECTION: All the data were selected firstly and examined for the references of each article. Collecting standard: ①The content of the articles should contain the research of using BMSC transplantation in treatment of ischemic brain damage.②The articles in the same field published recently or in authorized magazine were preferred. Exclusion standard: repeated study or Meta-analysis articles. DATA EXTRACTION: We collected 208 related articles, at last 42 articles met the inclusion criteria were collected. Other 166 papers were eliminated due to the old or repeated content of articles. In the collected 42 articles: 17 articles involved the definition and the biology characteristic of BMSC, 12 involved the nerve differentiation potential of BMSC, 9 involved the possible mechanism of BMSC nerve differentiation, 14 involved the transplant way of BMSC, and 15 involved the treatment function of BMSC to ischemic brain damage. DATA SYNTHESIS: How to improve the regeneration of nerve cells and revival of neurobehavior after brain damage is a puzzle to perplex the doctors in Department of Neurology. It has been proved by researchers that BMSC transplantation is available and feasible in treatment of focal cerebral ischemia. There are some BMSCs in the medulla of adult animals, and their differentiation towards neural stem cells can be realized under appropriate conditions. Some breakthroughs have been achieved in the induced differentiation of BMSC to neuron-like cells. It is extremely important to prolong the life of BMSC-born neuron-like cells. Additionally, some achievements have been made in the treatment of ischemic brain damage by BMSC transplantation. Much better results will be obtained if BMSCs are firstly differentiated to neuron-like cells in vitro and then transplanted. CONCLUSION: A series of breakthroughs have been made in the treatment of ischemic brain damage by BMSC transplantation, but the action mechanism and clinical application needs to further research.
