全固态绿光激光器与前列腺选择性光汽化术的应用与发展
Application and development of all solid state green laser and photoselective vaporization of prostate
【摘要】 目的:就近年来国内外有关前列腺良性增生症的治疗方法以及研究近况,综述选择性光汽化术治疗前列腺良性增生方面的研究进展。方法:主要选择被Medline收录的外文文献,就其内容进行分析、归纳及总结。结果:治疗良性前列腺增生的方法按出现的时间先后主要有:经尿道前列腺电切术、前列腺激光切除术、钬激光前列腺剜除术、前列腺光选择性汽化术。前列腺选择性光汽化术是治疗前列腺增生的优先选择。532nm绿色激光用于前列腺选择性光汽化术的优势在于其作用在组织上极高的激光能量。除汽化作用外,激光束在组织上形成很薄的凝固带,凝固带的扩展限制在1~2mm内,有很好的止血作用。系统介绍了前列腺选择性光汽化术以及其核心部件——全固态绿光激光器的原理与发展过程,并且对临床如何采用前列腺选择性光汽化术治疗前列腺良性增生进行了详细的介绍。结论:前列腺选择性光汽化术是治疗前列腺增生症的一种重要方法,采用532nm绿色激光用于治疗前列腺良性增生具有特殊优势。
【Abstract】 AIM:To review the progress in international researches on photoselective vaporization in the treatment of benign prostatic hyperplasia(BPH) based on the latest research of reconstruction for photo selective vaporization of the prostate in recent years.METHODS:English literatures in Medline were mainly selected,which were analyzed,summarized and reviewed.RESULTS:Treatments for BPH were:transurethral resection of prostate(TURP) ,laser resection of the prostate,holmium laser enucleation of the prostate(HoLEP) and photoselective vaporization of the prostate,and last method was better in curing prostatic hyperplasia before treatment.The predominance of photoselective vaporization by using 532 nm green laser was that it had high laser energy in working on tissues.Besides vaporization,a thin coagulation band caused by laser beam formed on the tissues,and the extension was limited within 1-2 mm with better hemostasis.In this article,photoselective vaporization of the prostate and the principle as well as development of its core components,all-solid-state green laser,were introduced systematically.Besides,it was greatly introduced how to apply photoselective vaporization of the prostate in the treatment of prostatic hyperplasia in clinic.CONCLUSION:Photoselective vaporization of the prostate is an important method of curing BPH.And there are special advantages by using 532 nm Green Light to cure BPH.
