【摘要】 目的通过对初学超声乳化白内障吸除术中所出现问题的回顾,分析基层医院白内障手术转型的可行性和注意事项。方法对笔者最初进行的50眼超声乳化白内障吸除术病例资料回顾,分析术中术后最常发生的并发症出现原因及如何预防。结果42例50眼,术后三月最佳矫正视力<0.1者1眼,0.1～0.3者8眼,0.4～0.6者21眼,≥0.8者20眼,与同期进行的晶体囊外摘除术比较,角膜水肿(22%)后囊破裂(14%)发生率较高,术后第一天视力不如已经熟练掌握的囊外摘除术。结论对有白内障囊外摘除经验的医生,过渡到超声乳化白内障吸除术是安全可行的,最初选择合适的病例,掌握撕囊、碎核技术,预防后囊破裂及处理好玻璃体脱出是初学超乳技术的关键。更多还原

【Abstract】 Objective Feasibilities and precautions on transition from conventional cataract operation to Phacoemulsification were summarized for beginners in floor hospitals based on the study and review over the initiatives. Methods Reviewing the author’s forepart 50 eyes phacoemulsification data, the most frequent concurrent and post operation syndromes were reasoned and preventions were stated. Results Among 42 out of 50 eyes for the best corrected visual acuity 3 months after operation, 1 eye<0.1, 8 eyes within 0.1～0.3, 21 eyes within 0.4～0.6, 20 eyes=0.8, which demonstrated better visual acuity recovery compared with cases by extracapsular cataract extraction in the same period. But higher occurrence of cornea edema and posterior capsular rupture were found. Besides, the sight on the immediate next day after operation was poorer than that by fully skilled extracapsular cataract extraction. Conclusions It’s securely feasible for the skilful ophthalmologists to transit from extracapsular cataract extraction to phacoemulsification. The key elements to phacoemulsification beginners are appropriate case option, technique management on continuous circular capsulectomy and nucleus smash, posterior capsular rupture prevention and treatment on vitreous prolapsed.更多还原
