胃癌间质中肿瘤相关纤维母细胞CD34和SMA的表达及意义

【摘要】 目的探讨胃癌间质中肿瘤相关纤维母细胞CD34和SMA的表达及与胃癌侵袭性的关系。方法应用免疫组织化学PV6000通用型二步法检测75例胃癌组织及10例正常胃黏膜组织中间质纤维母细胞CD34和SMA的表达,并分析其与胃癌临床病理特征的关系。结果CD34在正常胃黏膜组织纤维母细胞中阳性表达率为80.0%(8/10),而在胃癌组织纤维母细胞中仅6/75例呈阳性表达(8.0%),两者比较差异有显著性(P<0.01)。胃癌组织纤维母细胞中SMA阳性表达率为90.7%(68/75),显著高于正常胃黏膜组(30.0%)(P<0.01)。胃癌组织纤维母细胞中SMA表达与胃癌的Lauren分型及组织学分级无关(P>0.05),与临床分期和淋巴结转移有关(P<0.05),Ⅲ~Ⅳ期强阳性率(75.6%)明显高于Ⅰ~Ⅱ期(26.7%)(P<0.01),有淋巴结转移组的强阳性率(78.7%)明显高于无淋巴结转移组(17.9%)(P<0.01)。结论与正常组织中纤维母细胞相比,胃癌中纤维母细胞表现为CD34(+)表达的缺失,而转变为SMA(+)的肌纤维母细胞的特征,两者的联合检测将可能作为肿瘤相关纤维母细胞较特异的标记;肿瘤相关纤维母细胞与胃癌的侵袭转移密切相关。

【Abstract】 Purpose To study the expression of CD34 and SMA in tumor-associated fibroblasts of gastric carcinoma and its relationship with tumor invasion.Methods Immunohistochemical technique was used to detect the expression of CD34 and SMA in stromal fibroblasts of 75 cases of gastric carcinoma and 10 normal gastric tissues,and its relationship with clinical characteristics was analyzed.Results In 10 normal gastric tissues and 75 cases of gastric carcinoma,positive rates of CD34 in fibroblasts were 80.0% and 8.0%,and positive rates of SMA were 30.0% and 90.7%,respectively.There was statistical significance in expression of CD34 and SMA between gastric carcinoma and normal gastric tissue(P<0.01).Although the expression of SMA had no obvious relationship with the Lauren typing and the histological grade,it was closely associated with the clinical stage and lymph node metastases.The expression of SMA was higher in stages Ⅲ~Ⅳ than stagesⅠ~Ⅱ patients,and higher in the patients with lymph node metastases than without.Conclusion The loss of CD34 and gain of SMA expression in the tumor-associated fibroblasts are related to tumor invasion,and combined detection of CD34 and SMA may be a specific maker of tumor-associated fibroblasts.
