哮喘患者IL-18、IL-12及IFN-γ水平的初步研究

【摘要】 目的观察哮喘患者血清中IL-18、IL-12及INF-γ水平的变化及糖皮质激素对其的影响。方法采用ELISA法分别检测中度哮喘急性发作期患者(30例)口服强的松治疗1周前后和健康对照组者(20例)血清中IL-18、IL-12及INF-γ水平,并同时测1秒钟用力呼吸容积占预计值的百分比(FEV1%)和气道阻力占预计值的百分比(R5%)。结果IL-18水平急性发作期治疗前较治疗后、健康对照组为高(P<0.01)。IL-12,IFN-γ水平分别在急性发作治疗前较治疗后、健康对照组为低(P<0.01)。FEV1%下降和R5%升高(P<0.01)。直线相关分析表明,血清中IL-18与FEV1%、IL-12、IFN-γ呈负相关,与R5%呈正相关,血清中IFN-γ、IL-12与FEV1%呈正相关,与R5%呈负相关。结论哮喘患者血清IL-18水平升高,IL-12、IFN-γ水平下降,糖皮质激素可使IL-18水平下降,IL-12、IFN-γ水平升高。

【Abstract】 Objective To observe the changes of serum IL-18, IL-12 and IFN-γ and lung function in asthmatic patients and to evaluate the influence of glucocorticoid on the patients. Methods The serum samples were obtained from 30 asthmatic patients with acute asthma attack one week before and after course of oral prednisone, and 20 healthy volunteers. Serum IL-18, IL-12 and IFN-γ were determined with sandwich ELISA. Lung ventilatory function forced expiratory volume in one second (FEV1) and respiratory impedance airway resistance (R5) were measured in all patients. Results (1) Serum level of IL-18 in the asthma attack group was significantly higher than that in the normal control group after prednisone treatment (P<0.01); (2) Serum level of IL-12, IFN-γ in the asthma attack group were significantly lower than that in the normal control group after prednisone treatment (P<0.01); Correlation analysis showed that the serum level of IL-18 was negatively correlated to FEV1%, IL-12, IFN-γ, positively correlated to R5%; the serum level of IFN-γ, IL-12 was positively correlated to FEV1% and negatively correlated to R5%. Conclusion The secretion of IL-18 and IL-12、IFN-γ was impaired in asthma with a significant increase in serum level of IL-18 and decrease in serum level of IL-12, IFN-γ. Glucocorticoid could downregulate the serum level of IL-18 and upregulate the serum level of IL-12, IFN-γ, and improve lung function in asthmatic patients.
