门控心肌断层显像的临床应用
The Clinical Use of Gated Myocardial Perfusion Single Photon Emission Computed Tomography
【摘要】 目的探讨门控心肌断层显像在临床的应用价值。方法67例受检者,其中正常组10例,心肌梗死组38例,扩张型心肌病组19例,分别行静息门控心肌断层显像,并与平衡法门控心血池显像和彩色多普勒超声心动图进行比较。结果心肌梗死者梗死区心肌放射性分布减低,室壁运动和室壁增厚率明显降低;扩张型心肌病患者左室心腔扩大,心肌壁变薄,室壁运动和室壁增厚率弥漫性减低。心肌梗死组和扩张型心肌病组各项心功能参数较正常组明显降低;门控心肌断层显像测得的心功能参数与平衡法门控心血池显像和彩色多普勒超声心动图的检查结果有很好的相关性。结论门控心肌断层显像可以通过一次性显像,获得多方面信息,较常规心肌断层显像有更好的临床应用价值与社会经济效益。
【Abstract】 Objective To access the clinical value of gated myocardial perfusion single photon emission computed tomography(G-SPECT).Methods G-SPECT using 99mTc-MIBI was performed on 67 cases,including 10 normal cases,38 patients with myocardial infarction and 19 with dilated cardiomyopathy,in comparison with equilibrium radionuclide angiocardiography and echocardiography within one week. Results There were severe defects in the regions of myocardial infarction with prominent decrease of wall motion and thickness.Left ventricular cavity dilations and diffuse decreases of wall motion and thickness were seen in dilated cardiomyopathy.The differences of parameters of left ventricular function from G-SPECT were significant between the normal cases and the two groups of patients,and the values measured from G-SPECT had an excellent correlation with those from equilibrium radionuclide angiocardiography and echocardiography.Conclusion G-SPECT can demonstrate myocardial perfusion and left ventricular function.The results of the parameters of ventricular function are reliable.It has a better clinical application and social economic effect than myocardial perfusion imaging.
