【摘要】 [目的]探讨带血管蒂股前外侧筋膜辩治疗股骨头缺血性坏死的疗效。[方法]14例(16个髋关节)股骨头缺血性坏死患者,男10例,女4例;年龄17～49岁,平均35.5岁。根据世界骨循环研究会(ARCO)国际骨坏死分期标准(1997):Ⅱ-A期1髋,Ⅱ-B期3髋;Ⅱ-C期4髋;Ⅲ-A期5髋,Ⅲ-B期3髋;用带旋股外侧动脉血管为蒂的股前外侧筋膜瓣移位包绕坏死的股骨头。对14例病例(16个髋关节),从跛行、疼痛、功能及X线表现4个方面进行髋关节功能评定。[结果]术后所有病例经2年9个月～4年5个月,平均3年4个月的随访,按改良的新疗效标准评定:Ⅱ-A期1髋:优1髋;Ⅱ-B期3髋:优2髋,良1髋;Ⅱ- C期4髋:优2髋,良2髋;Ⅲ-A期5髋:优2髋,良1髋,改善1髋,差1髋;Ⅲ-B期3髋:良2髋,差1髋。优良率81.25%。[结论]带血管蒂股前外侧筋膜瓣治疗股骨头缺血性坏死,具有创伤小、取材方便、并发症少、费用低、远期效果好等优点。更多还原

【Abstract】 [Objective]To Study the curative results of femur head necrosis treated by transfer of vascularized fascial flaps of antero- lateral femoral,[Methods]There are 14 cases(16 hip joints)of femur head necrosis,10 males and 4 females with mean age 35.5 years(range 17～49 years).According to clinic type of ARCO(1997):Ⅱ-A 1 hip joint,Ⅱ-β3 hip joints:Ⅱ-C 4 hip joints;Ⅲ-A 5 hip joints,Ⅲ-B 3 hip joints.All cases were planed and covered with vascularized fascial flaps of anterolateral femoral.To assess of function with the relief of pain and recovery of active and function of the hip and lame and X-ray.[Results]All patients were fol- lowed up for 2.9～4.5 years with an average of 3.4 years.According to the assessment of new standard,Ⅱ-A for excellent 1 hip joint;Ⅱ-B for excellent 2 hip joints and good 1 hip joint;Ⅱ-C for excellent 2 hip joints and good 2 hip joints;Ⅲ-A for excellent 2 hip joints and good 1 hip joint,fair 1 hip joint,poor 1 hip joint;Ⅲ-B for good 2 hip joints and poor 1 hip joint.The excellent and good rate is 81.25 percent.[Conclusion]This surgical method is easily to operate,with less damage to normal joint structure,less complication,less cost much and long term outcome.更多还原
