单臂半环式三维固定胫腓骨骨折支架的生物力学实验及临床应用
BIOMECHANICAL EXPERIMENT AND CLINICAL APPLICATION OF SINGLE-ARM SEMICIRCULAR THREE-DEMENSIONAL FIXER FOR TIBIO-FIBULAR FRACTURE
【摘要】 为观察自制单臂半环胫腓骨骨折固定器治疗胫腓骨骨折的生物力学效应和临床疗效,选择成人离体新鲜骨1具,新鲜牛胫腓骨4具(其中2具备用),分别制作成斜形、横断形、粉碎性骨折模型,以骨折复位器模拟临床治疗方法将之复位固定后,固定于电子万能试验机上,分别对3种模型模拟骨折固定后的6种受力状态(施加轴向拉伸力、轴向压缩力、前后向弯曲压力、侧向弯曲压力、左扭转力、右扭转力)进行试验,观察骨折端位移,分别记录试验数据、坐标曲线图。同时对50例胫腓骨骨折患者进行临床观察。结果显示,对骨折复位固定后的模型施加较常规外力可增加数倍的力,骨折复位固定器及骨折端本身的位移数均在许可范围。经50例临床应用观察,骨折全部愈合。表明该复位固定器设计力学结构合理,无偏心应力,具有弹性固定功能,性能稳定,能有效维持骨折复位效果的作用。且操作简便、固定牢靠、实用。
【Abstract】 The objective of the paper is to observe the biomechanical and clinical effect of self-made single-arm semicircular three-dimenssion fixer(SASF) on tibio-fibular fracture(TFF).A fresh human tibia from adult and four pairs of bovine tibia and fibula(two of them stored for use) were made into the test samples of oblique,transverse and comminuted fracture.After reduced and fixed by SASF,the samples were put on electronic universal test machine and experimented under six loads: axial extension,axial compression,anteroposterior incurvation,lateral incurvation,left torsion and right torsion;the fragment displacements were observed;the test data and coordinate curve diagrams were recorded.At the same time,the clinical observations were made on 50 cases of TFF treated by SASF.The results showed that after the reduction and fixation by SASF,several times of the conventional external forces were exerted on the test samples,the displacement values of SASF and the fragments remained in the permitted range;the observation on SASF clinical application showed that all of 50 cases obtained fracture healing.【
