预变性周围神经移植和神经生长因子灌注对脊髓损伤后上行传导束再生的影响
Experimental study on regeneration of ascending tract due to predegenerated peripheral nerve graft and nerve growth factor infusion
【摘要】 目的:评价应用预变性周围神经移植(PPNG)和神经生长因子(NGF)灌注对大鼠脊髓损伤(SCI)后上行传导束再生的影响及两者协同效果。方法:50只SD大鼠分5组,每组10只,A组为PPNG和NGF灌注组;B组为PPNG组;C组为NGF灌注组;D组为单纯损伤组;E组为正常对照组。A、B组于脊髓损伤前1周取坐骨神经约3mm并置于DMEM中。1周后造成T10脊髓后索损伤并将预变性的外周神经植入,A、C组脊髓损伤后置管注射NGF,每日2μg,共2周;D组单纯损伤脊髓,E组不损伤脊髓。8周后行辣根过氧化物酶(HRP)示踪法计数再生的纤维,按照脊髓与移植物的关系分为脊髓尾端、尾端过渡区、移植物尾端、移植物头端、头端过渡区、脊髓头端六个区,简称Ⅰ、Ⅱ、Ⅲ、Ⅳ、Ⅴ、Ⅵ区。结果:在Ⅲ、Ⅳ、Ⅴ区,A组标示的纤维数量显著高于B、C组(P<0.05),Ⅵ区均无示踪纤维;D组几无纤维通过移植物。结论:联合应用PPNG及NGF灌注可明显促进大鼠SCI后上行传导束的再生,使再生的纤维跨过两道瘢痕,重新长入脊髓白质内。
【Abstract】 Objective:To explore the effect of predegenerated peripheral nerve graft combined with nerve growth factor(NGF) infusion on ascending sensory tract regeneration after spinal cord injury and evaluate the coordinative effect of the two ways.Method:50 female SD rats were divided into 5 groups randomly.Group A was treated with PPNG and NGF infusion,group B with PPNG,group C with NGF infusion,group D and E as blank and normal control.A segment of sciatic nerve was dissected out and kept in DMEM for 1 week before implantation.One week later,dorsal funiculus at T10 was transected and removed.Immediately the predegerated PNG was transplanted and the NGF infusion device was implanted.2μg NGF was infused everyday.8 weeks later,horseradish peroxidase-labbled(HRP) tracing method was used to evaluated the regeneration of 5 groups.The extent of regeneration in and beyond the nerve graft was determined by counting the number of HRP-labeled fibers intersecting imaginary lines placed perpendicular to the axis of the graft and cord.For the sake of convenience,according to the relation of the PNG and spinal cord,6 model zones were divided caudal of spinal cord,caudal transition zone,caudal zone in graft,rostral zone in graft,rostral transition zone and rostral of spinal cord.Result:At the transverse section of caudal zone in graft,rostral zone in graft,rostral tranzition zone,the fibers amount of group A are higher than group B and C which has significant difference(P<0.05).Few cross fiber was found in group D.Conclusion:With the coordination of PPNG and NGF,the ascending long tract regeneration after spinal cord injury were significantly promoted.The regenerative fibers can penetrate the two graft-host interface scars.
