北京市2005年接种吸附百白破联合疫苗后107例无菌化脓流行病学分析

【摘要】 目的了解北京市2005年接种吸附百白破联合疫苗(DPT)后无菌化脓病例增多的原因,保证安全接种。方法对北京市2005年接种DPT后107例无菌化脓进行描述流行病学分析。结果北京市2005年共报告107例无菌化脓,发生率为19.58/10万人次。83%(15/18)的区(县)报告了无菌化脓,共涉及6个批号DPT,其中80%的病例涉及2个批号,发生率分别为27/10万人份、15/10万人份。结论北京市2005年接种DPT后无菌化脓病例增多主要原因可能为:①DPT更换生产厂家,部分批号DPT的质量可能存在问题;②预防接种不良反应监测系统敏感性提高。疫苗生产厂家应改善工艺、提高质量,建议使用无细胞百白破联合疫苗。

【Abstract】 Objective To understand the cause for the increase of sterile abscess cases upon DPT(whole cell) vaccination in 2005 in Beijing,so as to control the AEFI rate and improve vaccination safety.Method Epidemiological analysis was carried out in 107 reported sterile abscess cases upon DPT(whole cell) vaccination in 2005 in Beijing.Results 107 sterile abscess cases upon DPT(whole cell) vaccination were reported in 2005 in Beijing,with an incidence of 19.58 per 100,000.83.33%(15/18) of all the districts and counties of Beijing reported the sterile abscess cases,with an incidence of 0-96.69 per 100,000.6 DPT lot numbers were involved,with 2 lot numbers responsible for 80.00% of all the sterile abscess cases.And the incidence rates were 27.00 per(100,000) and 15.00 per(100,000),respectively.No procedure errors were found in the vaccination process for all the above sterile abscess cases.Conclusion The cause for the increase of sterile abscess cases upon DPT(whole cell) vaccination in 2005 in Beijing might be the shift in DPT providers and the enhanced sensitivity in the AEFI monitoring system in Beijing.The DPT manufacturers should focus on improvement of the vaccine quality.And acellular DPT is recommended in the vaccination.
